      摘 要:[目的] 探讨短串联重复序列(short tandem repeat,STR)基因多态性检测在部分性葡萄胎鉴别诊断中的临床应用价值。[方法] 收集济宁医学院附属金乡医院病理科2005~2016年临床及病理形态学上明确部分性葡萄胎和水肿性流产各20例。组织标本经STR基因多态性检测,STR检测包括9个多态性位点(D7S820、D13S317、D16S59、TPOX、TH01、CSF1PO、VWA、F13A01、FESFPS),分析其在葡萄胎鉴别诊断中的作用。[结果] 水肿性流产患者中位年龄27岁(23~33岁),部分性葡萄胎患者中位年龄30岁(25~37岁)。组织病理学结果:水肿性流产可见局灶绒毛组织水肿,滋养叶细胞增生不明显;部分性葡萄胎局灶绒毛组织水肿,中度滋养叶细胞增生。STR结果显示:水肿性流产在每个STR位点均含有父源性和母源性各1个等位基因(17/17),另外3例未检测成功。在部分性葡萄胎中,单卵单精型部分性葡萄胎和单卵双精型部分性葡萄胎分别为3(3/19)例和12例(12/19),还有4例为均含有父源性和母源性各1个等位基因(4/19),另有1例未检测成功。[结论] 传统的组织病理学对部分性葡萄胎的诊断准确性仍然不够,9个STR位点多态性分析对部分性葡萄胎的诊断具有重要作用。

      Abstract:[Objective] To assess the application of short tandem repeats(STR) analysis in the diagnostic of partial hydatidiform mole(PHM). [Methods] Twenty samples of partial hydatidiform mole and 20 samples of hydropic abortus were collected from the Department of Pathology,Affiliated Jinxiang Hospital of Jining Medical College from 2005 to 2016. The STR analysis was performed in seven loci(D7S820,D13S317,D16S59,TPOX,TH01,CSF1PO,VWA,F13A01,FESFPS) on the tissue samples and their values for differential diagnosis of hydatidiform mole were assessed. [Results] Histopathologically,there was focal villous swelling and no obvious trophoblastic hyperplasia in hydropic abortus; while partial hydatidiform mole(PHM) was characterized by moderate trophoblastic hyperplasia and hydatidiform villous swelling. Three cases of hydropic abortus failed for STR detection,and STR analysis showed balanced biallelic profiles of both paternal and maternal origins in all 17 cases. In 20 PHM cases,1 case failed for STR analysis; 3 were monospermic partial hydatidiform mole(MPM,3/19),12 were dispermic partial hydatidiform mole(DPM,12/19),and 4 cases showed balanced biallelic profiles of both paternal and maternal origins(4/19). [Conclusion] The histopathologic diagnosis of partial hydatidiform mole is less accurate. STR analysis with 9 sites is of value in the diagnosis of partial hydatidiform mole.
