| 摘 要：［目的］ 分析促性腺激素释放激素激动剂（GnRHa）戈舍瑞林(goserelin)联合腹腔镜保守手术对子宫内膜异位症的疗效。［方法］ 将389例育龄期Ⅲ/Ⅳ期子宫内膜异位症患者随机分为3组：一组腹腔镜保守手术后皮下注射戈舍瑞林3.6mg治疗3个月（n=206），一组同样于手术后皮下注射戈舍瑞林3.6mg治疗6个月（n=82），对照组术后无其他治疗(n=101)。随访术后18个月子宫内膜异位症复发和妊娠率的情况。［结果］ 戈舍瑞林治疗6个月和3个月组患者的子宫内膜异位症复发率(χ2=6.513，P=0.035)差异有统计学意义，两组间相关不孕症患者妊娠率差异无统计学意义（P=0.263），而戈舍瑞林治疗6个月组和3个月组患者与对照组相比，妊娠率差异均有显著统计学意义(P<0.05)。［结论］ Ⅲ/Ⅳ期子宫内膜异位症患者腹腔镜保守手术术后应用戈舍瑞林，能显著提高患者妊娠率，同时延长戈舍瑞林治疗时间能更好地预防子宫内膜异位症复发。 |

| Abstract：［Purpose］ To investigate the effect of gonadotropin-releasing hormone agonist goserelin combined with conservative laparoscopic surgery in the treatment of endometriosis.［Methods］Three hundred and eight-nine childbearing age women with endometriosis stages Ⅲ or Ⅳ were divided into three groups：206 patients received 3 months goserelin 3.6mg i.h. after conservative laparoscopic surgery;82 patients received 6 months goserelin 3.6mg i.h. after conservative laparoscopic surgery; and 101 patients with conservative laparoscopic surgery alone. Endometriosis relapse and pregnancy rate were evaluated during the following 18 months. ［Results］ There was significant difference in endometriosis relapse between patients treated with 6 months GnRHa and 3-months GnRHa(χ2=6.513，P=0.035)，and no significant difference was found in the pregnancy rate between the two groups(P=0.263). But there were significant differences in the pregnancy rate in either of the goserelin groups compared with control group(P<0.05). ［Conclusion］ GnRHa treatment after conservative laparoscopy for stage Ⅲ/Ⅳ endometriosis might improve the pregnancy rate in endometriosis-associated infertility. Prolonging goserelin therapy might effectively prevent the relapse of endometriosis. |
