| 摘 要：［目的］ 探讨心理干预治疗对乳腺癌患者改良根治手术后焦虑抑郁情绪的影响。［方法］ 对2007年10月至2010年10月住院的84例乳腺癌患者在入院后3天内手术前进行汉密尔顿抑郁量表（HAMD）和汉密尔顿焦虑量表（HAMA）评估，并随机分为研究组（42例）和对照组（42例）。研究组采用心理社会干预治疗，对照组采用一般健康宣教，观察6个月，比较两组术后1个月和6个月的情绪变化。［结果］ 术后1个月研究组HAMD和HAMA减分均大于对照组（HAMD：6.4±1.5 vs 2.8±1.2，F=12.15，P＜0.05，HAMA：5.6±1.1 vs 2.2±1.2，F=14.33，P＜0.05），术后6个月干预结束时研究组HAMD和HAMA减分也均大于对照组（HAMD：12.0±1.1 vs 6.2±1.8，F=17.81，P＜0.05；HAMA：9.0±2.1 vs 4.4±1.3，F=12.07，P＜0.05）。术后6个月研究组患者焦虑缓解率（71.4%）高于对照组（38.1%），差异有统计学意义（?字2=9.42，P＜0.05），抑郁情绪缓解率（59.5%）也高于对照组（33.3%）（?字2=5.79，P＜0.05）。［结论］ 心理社会干预能有效缓解乳腺癌患者根治术后的焦虑抑郁状态。 |

| Abstract：［Purpose］ To investigate efficacy of psychological intervention in patients with breast cancer postoperation. ［Methods］ All breast cancer patients were assessed according to HAMD and HAMA scores before surgery，and then randomly divided into study group(n=42) and control group(n=42). Study group treated with modified radical surgery combined with psychological intervention and control group treated with modified radical surgery only. The efficacy was assessed 1 month and 6 months after surgery.［Results］ Compared with the control group，HAMD and HAMA score of study group significantly decreased 1 month after surgery（HAMD：6.4±1.5 vs 2.8±1.2，F=12.15，P＜0.05；HAMA：5.6±1.1 vs 2.2±1.2，F=14.33，P＜0.05）. Both HAMD and HAMA score of the study group significantly decreased 6 month after surgery（HAMD：12.0±1.1 vs 6.2±1.8，F=17.81，P＜0.05；HAMA：9.0±2.1 vs 4.4±1.3，F=12.07，P＜0.05）.The remission rates for anxiety and depression were both higher in study group than those in the control group(71.4% vs 38.1%，59.5% vs 33.3%，P＜0.05).［Conclusion］ Psychological intervention is effective in the treatment for depression and anxiety in patients with breast cancer. |
