| 摘 要:[目的] 探讨甲状腺乳头状癌(papillary thyroid carcinoma, PTC)中基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)和碱性成纤维细胞生长因子(basic fibroblast growth factor, bFGF)的表达及意义。[方法] 收集2024年10月至2025年8月期间青岛大学附属医院手术治疗的60例PTC患者的癌组织标本及相应的癌旁甲状腺组织标本。采用免疫组织化学检测60例患者组织中MMP-9和bFGF蛋白的表达定位及半定量分析;并采用20例患者的新鲜组织标本,应用免疫印迹法检测MMP-9和bFGF蛋白的表达水平。采用SPSS 26.0统计软件分析MMP-9和bFGF蛋白表达与PTC患者临床病理特征间的关系,并采用Spearman相关分析两者表达相关性。[结果] 免疫印迹法检测结果显示,MMP-9和bFGF在PTC组织中的相对表达量均显著性高于癌旁组织(MMP-9:1.57±0.17 vs 0.32±0.11,t=28.14,P<0.001;bFGF:1.21±0.11 vs 0.27±0.09,t=28.87,P<0.001)。免疫组织化学检测结果显示MMP-9和bFGF在PTC肿瘤组织中的阳性表达率分别为75.00%(45/60)、78.33%(47/60),显著性高于对应癌旁组织的20.00%(12/60)和23.33%(14/60);MMP-9、bFGF阳性表达均与PTC患者的淋巴结转移及TNM分期显著性相关(P<0.001),而与患者性别、年龄、肿瘤大小无显著性相关(P均>0.05)。PTC组织中MMP-9与bFGF表达呈正相关(r=0.74,P<0.001)。[结论] MMP-9与bFGF在PTC组织中均呈高表达,且两者表达协同上调,与PTC的淋巴结转移和临床分期相关,可能共同促进PTC的侵袭与转移。MMP-9与bFGF可作为评估PTC生物学行为及预后的潜在分子标志物。 |

| Abstract: [Objective] To investigate the expression of matrix metalloproteinase-9 (MMP-9) and basic fibroblast growth factor (bFGF) in papillary thyroid carcinoma (PTC), and to evaluate their clinical significance. [Methods] A total of 60 paired PTC and adjacent thyroid tissue specimens were collected from patients who underwent surgery at The Affiliated Hospital of Qingdao University between October 2024 and August 2025. Immunohistochemistry was used to determine the localization and semi-quantitative expression of MMP-9 and bFGF proteins in all 60 paired tissues. Western blot was additionally performed on fresh tissue samples from 20 patients to quantify the protein expression levels. The associations between MMP-9/bFGF expression and clinicopathological features were analyzed using SPSS 26.0, and the correlation between the two markers was assessed by Spearman’s correlation test. [Results] Western blot showed that the relative expression levels of MMP-9 and bFGF were significantly higher in PTC tissues than those in adjacent tissues(MMP-9: 1.57±0.17 vs 0.32±0.11, t=28.14, P<0.001; bFGF: 1.21±0.11 vs 0.27±0.09, t=28.87, P<0.001). Immunohistochemistry revealed that the positive expression rates of MMP-9 and bFGF in PTC tissues were 75.00% (45/60) and 78.33% (47/60), respectively, which were significantly higher than those in adjacent tissues [20.00% (12/60) and 23.33% (14/60), respectively]. Positive expression of both proteins was significantly correlated with lymph node metastasis and TNM stage(all P<0.001), but not with sex, age, or tumor size (all P>0.05). Spearman correlation analysis demonstrated a positive correlation between MMP-9 and bFGF expression in PTC tissues (r=0.74, P<0.001). [Conclusion] Both MMP-9 and bFGF are highly expressed in PTC tissues and show synergistic upregulation. Their expression is associated with lymph node metastasis and clinical stage, suggesting that they may jointly promote the invasion and metastasis of PTC. They could serve as potential molecular markers for evaluating the biological behavior and prognosis of PTC. |
