| 摘 要： 腹盆腔淋巴结转移是宫颈癌预后的独立危险因素，其标准治疗是以顺铂为基础的同步放化疗，腹主动脉旁淋巴结转移患者可考虑延伸野照射以改善生存，但需平衡其增加正常组织损伤风险。淋巴结切除术的生存获益缺乏前瞻性随机对照证实，且与放疗联合可能增加并发症。腹膜后淋巴结转移患者可能从辅助化疗中获益，免疫治疗联合同步放化疗为高危患者治疗提供了新选择。靶向治疗药物尼妥珠单抗可推荐用于老年或不耐受化疗的局部晚期宫颈癌患者，其他靶向药物的疗效与安全性仍需进一步研究。全文就伴腹盆腔淋巴结转移宫颈癌综合治疗现状进行分析总结，为临床诊疗提供参考。 |

| Abstract： Abdominopelvic lymph node metastasis is an independent risk factor for the prognosis of cervical cancer. The standard treatment is cisplatin-based concurrent chemoradiotherapy. For patients with para-aortic lymph node metastasis， extended-field irradiation may be considered to improve survival， although this must be balanced against the increased risk of normal tissue toxicity. The survival benefit of lymphadenectomy lacks confirmation from prospective randomized trials， and its combination with radiotherapy may increase complications. Patients with retroperitoneal lymph node metastasis may benefit from adjuvant chemotherapy. Immunotherapy combined with concurrent chemoradiotherapy offers a new treatment option for high-risk patients. The targeted agent nimotuzumab can be recommended for elderly patients or those with locally advanced cervical cancer who are intolerant to chemotherapy； however， the efficacy and safety of other targeted drugs require further investigation. This review analyzes and summarizes the current status of comprehensive treatment for cervical cancer with abdominopelvic lymph node metastasis， providing a reference for clinical practice. |
