| 摘 要:胃癌是常见恶性肿瘤,其预后依赖准确的早期诊断和动态检测。鉴于胃癌高度分子异质性的特性,传统组织活检受肿瘤空间和时间异质性限制,难以全面反映疾病进展。近年来,循环肿瘤细胞(circulating tumor cells,CTCs)和循环肿瘤DNA(circulating tumor DNA,ctDNA)检测技术的开展为胃癌的早期筛查、病程追踪和精准治疗提供了新方向。全文总结了CTCs、ctDNA在胃癌早期诊断、疗效和预后预测、复发和耐药机制探索等方面的临床应用。同时,探讨了CTCs和ctDNA联合检测的临床应用前景与挑战,期待其未来能成为临床实践中胃癌诊断和治疗的规则改变者。 |

| Abstract: Gastric cancer is a prevalent malignancy globally, for which patient outcomes depend critically on accurate early diagnosis and dynamic disease monitoring. Owing to the high degree of molecular heterogeneity in gastric cancer, traditional tissue biopsies are constrained by spatial and temporal limitations, hindering a comprehensive assessment of disease progression. Recent technological advances in the detection of circulating tumor cells (CTCs) and circulating tumor DNA (ctDNA) have opened new avenues for early screening, monitoring disease progression, and enabling precision therapy in gastric cancer. This review comprehensively summarizes the clinical utility of CTCs and ctDNA in the early detection of gastric cancer, evaluation of therapeutic efficacy, prognosis prediction, monitoring for recurrence, and investigation of drug resistance mechanisms. Additionally, it discusses the promising prospects and prevailing challenges associated with the combined analysis of CTCs and ctDNA, positing that this integrated approach holds the potential to be a transformative strategy in the management of gastric cancer. |
