| 摘 要:肿瘤性贫血(cancer-related anemia,CRA)是肿瘤患者常见的并发症,严重影响患者的生活质量和预后,其发病机制复杂。炎症因子可激活多种信号通路,上调铁调素,通过调控铁代谢、抑制促红细胞生成素信号通路及直接损伤骨髓造血功能等多种途径影响铁代谢和红细胞生成;基因突变(铁代谢相关基因突变、红细胞生成相关基因突变、DNA修复与骨髓抑制相关基因突变、炎症相关基因突变等)、肿瘤微环境异常以及信号通路异常等均与CRA发生有关。全文综述了CRA发病机制的最新研究进展,旨在为临床实践研究提供理论依据。 |

| Abstract: Cancer-related anemia (CRA) is a frequent complication in cancer patients, significantly impairing their quality of life and prognosis. The pathogenesis of CRA is multifaceted. Inflammatory cytokines play a central role by activating diverse signaling pathways, leading to the upregulation of hepcidin. This, in turn, disrupts iron homeostasis and erythropoiesis via several mechanisms: dysregulation of iron metabolism, suppression of the erythropoietin (EPO) signaling axis, and direct impairment of bone marrow hematopoietic function. Additionally, genetic alterations (involving genes related to iron metabolism, erythropoiesis, DNA repair, bone marrow suppression, and inflammation), aberrations in the tumor microenvironment, and dysregulated signaling pathways all contribute to the development of CRA. This review synthesizes recent advances in understanding the pathogenesis of CRA, with the aim of informing and guiding clinical practice. |
