| 摘 要：近年来，新辅助治疗后达到临床完全缓解（clinical complete response，cCR）的直肠癌患者采用等待观察策略已成为该领域的热点议题。等待观察策略不仅可以实现器官保留，还能减轻患者的心理和经济负担，从而提升生活质量。全文分析等待观察策略的适用人群、监测与随访、潜在风险与收益的权衡、个体差异的考量以及临床研究设计等，并强调未来研究应聚焦于优化等待观察策略的实施与评估。同时，多学科团队的协作、患者教育以及心理支持的整合对于提供更全面的患者管理方案至关重要。 |

| Abstract： The application of watch and wait (W&W) strategy for rectal cancer patients who achieve clinical complete response (cCR) after neoadjuvant therapy (NAT) has become a hot topic in recent years. The W&W strategy can not only achieve organ preservation， but also reduce the psychological and economic burden of patients， thereby improving the quality of life. This paper analyzes the applicable population， monitoring and follow-up， trade-off between potential risks and benefits， consideration of individual differences， and clinical research design of W&W strategy. The future research should focus on optimizing the implementation and evaluation of W&W strategies， and the collaboration of multidisciplinary teams， patient education and the integration of psychological support are essential for a comprehensive patient management program. |
