| 摘 要：［目的］ 评估早期宫颈癌患者根治性宫颈切除术的疗效，探讨相关的预后因素，并分析其生育结局和生活质量。［方法］ 纳入2009年1月至2023年12月在浙江省肿瘤医院接受保留生育功能手术的70例早期宫颈癌患者，随访观察肿瘤学和妊娠结局，并通过EORTC QLQ-CX24问卷评估生活质量。［结果］ 经保留生育功能手术治疗的早期宫颈癌患者中位生存时间为61.0个月，中位无病生存时间为60.5个月。23例有生育意愿患者中妊娠率为21.7%。生活质量分析显示患者对身体形象、性活动和性愉悦感的满意度较高，症状评分显示轻微影响。［结论］ 根治性宫颈切除术可为早期宫颈癌患者提供有效的生育功能保护与长期生存获益。优化手术技术、多学科协作及精准术后管理是改善生殖与肿瘤综合预后的关键。 |

| Abstract： ［Objective］ To evaluate the fertility outcomes and quality of life in early-stage cervical cancer patients following radical trachelectomy. ［Methods］ Seventy patients with early-stage cervical cancer who underwent fertility-sparing surgery at Zhejiang Cancer Hospital between January 2009 and December 2023 were included in the study. Oncological and pregnancy outcomes were monitored during follow-up， and quality of life was assessed by EORTC QLQ-CX24 questionnaire. ［Results］ The median overall survival was 61.0 months， and the median disease-free survival was 60.5 months. Among the 23 patients desiring fertility， the pregnancy rate was 21.7%. Quality of life analysis showed that patients reported high satisfaction with body image， sexual activity， and sexual enjoyment， with minimal symptomatic impact. ［Conclusion］ Radical trachelectomy offers long-term survival benefits and effective fertility preservation for patients with early-stage cervical cancer. |
