摘 要:[目的] 探讨Syncytin-1和 Cripto-1在非小细胞肺癌(non-small cell lung cancer,NSCLC)患者胸腔积液外泌体中的表达及其临床意义。[方法] 对41例NSCLC和41例肺炎患者的胸腔积液进行外泌体的提取、鉴定、RNA提取及Syncytin-1、Cripto-1表达的检测。 通过蛋白印迹法检测外泌体蛋白表达,并利用酶联免疫吸附实验检测外泌体内蛋白含量,分析Syncytin-1和Cripto-1在NSCLC患者胸腔积液外泌体中的表达量与临床病理特征的相关性,并评估其对NSCLC的诊断效能。[结果] 与肺炎患者相比,NSCLC患者的胸腔积液外泌体中Syncytin-1和Cripto-1的表达量升高(P<0.001)。蛋白印迹法检测显示外泌体表达其特异性蛋白。 Syncytin-1和Cripto-1在NSCLC患者组和肺炎患者组之间存在统计学差异(P<0.001)。Syncytin-1和Cripto-1表达量对NSCLC诊断灵敏度分别为82.9%和75.6%、特异度分别为75.6%和80.5%,曲线下面积(area under curve,AUC)分别为0.847和0.849。Syncytin-1和Cripto-1诊断NSCLC的AUC值高于神经元特异性烯醇化酶和胃泌素释放肽前体(AUC分别为0.762和0.716),但低于癌胚抗原(AUC=0.931)(P<0.001)。[结论] 胸腔积液外泌体中Syncytin-1、Cripto-1检测对于诊断NSCLC具有较高的灵敏度和特异度,可作为辅助检测NSCLC的新型分子标志物。 |

Abstract: [Objective] To explore the expression of Syncytin-1 and Cripto-1 in exosomes of pleural effusion in patients with non-small cell lung cancer(NSCLC) and their clinical significance. [Methods] The exosomes were extracted and identified from pleural effusion of 41 NSCLC and 41 pneumonia patients. The expression of Syncytin-1 and Cripto-1 in exosomes were detected with qRT-PCR. Western blotting was used to detect exosomal protein expression, and enzyme-linked immunosorbent assay (ELISA) was used to measure the contents of exoprotein. The association of Syncytin-1 and Cripto-1 expression in exosomes with clinicopathological features of NSCLC patients was analyzed, and the diagnostic efficacy of Syncytin-1 and Cripto-1 for NSCLC was evaluated. [Results] Compared with pneumonia patients, the expression levels of Syncytin-1 and Cripto-1 in exosomes of pleural effusion in NSCLC patients were significantly increased (P<0.001). Western blotting analysis detected specific proteins in exosomes. ELISA results showed that there were significant differences in the expression of Syncytin-1 and Cripto-1 in exosome between NSCLC patients and pneumonia patients(P<0.001). The area under the ROC curve(AUC) of Syncytin-1 and Cripto-1 for diagnosis of NSCLC were 0.847 and 0.849, with a sensitivity of 82.9% and 75.6%, specificity of 75.6% and 80.5%, respectively. The AUC values of Syncytin-1 and Cripto-1 were significantly higher than those of neuron-specific enolase and progastrin-releasing peptide (0.762 and 0.716), but lower than that of carcinoembryonic antigen (AUC=0.931) (P<0.001). [Conclusion] The expression of Syncytin-1 and Cripto-1 in pleural effusion exosomes exhibits high sensitivity and specificity for diagnosis of NSCLC, which may be used as novel molecular markers for auxiliary diagnosis of NSCLC. |
