      摘 要:[目的] 探讨食管鳞状细胞癌组织中长链非编码RNA(long non-coding RNA,lncRNA)HOTAIR表达的临床病理意义及其与上皮间质转化(epithelial-mesenchymal transition,EMT)的关系。[方法]选择2015年1月至2019年12月在重庆荣昌区人民医院手术治疗的85例食管鳞状细胞癌患者,收集食管鳞状细胞癌组织及对应的癌旁组织,检测lncRNA HOTAIR的表达水平以及EMT标志基因E-钙黏蛋白(E-cadherin)、N-钙黏蛋白(N-cadherin)及波形蛋白(Vimentin)的mRNA表达水平及蛋白高表达率。采用Pearson检验及Spearman检验分析lncRNA HOTAIR与EMT标志基因表达的相关性。随访食管鳞状细胞癌患者的总生存期、无进展生存期,采用Kaplan-Meier曲线分析lncRNA HOTAIR表达水平与总生存期、无进展生存期的关系。[结果] 食管鳞状细胞癌组织中lncRNA HOTAIR的表达水平、N-cadherin及Vimentin的mRNA表达水平及蛋白高表达率均高于癌旁组织(1.58±0.38 vs 1.00±0.28,1.39±0.40 vs 1.00±0.29,1.29±0.37 vs 1.00±0.30,81.18% vs 40.00%,68.24% vs 36.47%,P均<0.001),E-cadherin的mRNA表达水平及蛋白高表达率均低于癌旁组织(0.72±0.19 vs 1.00±0.21,23.53% vs 55.29%,P均<0.001)。lncRNA HOTAIR的表达水平与E-cadherin的mRNA表达水平及蛋白高表达率呈负相关,与N-cadherin、Vimentin的mRNA表达水平及蛋白高表达率呈正相关(P<0.05)。食管鳞状细胞癌组织中病灶最大径>4.5 cm、T3期、N1~2期组的lncRNA HOTAIR的表达水平高于病灶最大径≤4.5 cm、T1~2期、N0期组(1.71±0.49 vs 1.48±0.44,1.67±0.44 vs 1.43±0.52,1.68±0.40 vs 1.44±0.54,P均<0.05)。食管鳞状细胞癌组织中lncRNA HOTAIR表达水平≥中位数(1.57)患者的总生存率、无进展生存率均较lncRNA HOTAIR表达水平<中位数的患者短(P均<0.05)。[结论] 食管鳞状细胞癌中lncRNA HOTAIR表达增加与病理进展、预后不良有关,促进EMT可能是高表达的lncRNA HOTAIR参与食管鳞状细胞癌发病的相关分子机制。

      Abstract:[Objective] To investigate the clinicopathological significance of long non-coding RNA (lncRNA) HOTAIR expression in esophageal squamous cell carcinoma and its relationship with epithelial-mesenchymal transition(EMT). [Methods] Eighty-five patients with esophageal squamous cell carcinoma who underwent surgical treatment in Chongqing Rongchang District People’s Hospital from January 2015 to December 2019 were enrolled. The expression level of lncRNA HOTAIR and EMT marker genes E-cadherin, N-cadherin and Vimentin were detected in cancer tissue and corresponding adjacent tissue samples. Pearson test and Spearman test were used to analyze the correlation between lncRNA HOTAIR and EMT marker genes. The relationship of lncRNA HOTAIR expression with overall survival (OS) and progression free generation (PFS) of patients was analyzed. [Results] The expression level of lncRNA HOTAIR, the mRNA expression level and the high expression rate of N-cadherin and Vimentin in esophageal squamous cell carcinoma were higher than those in adjacent tissues (1.58±0.38 vs 1.00±0.28, 1.39±0.40 vs 1.00±0.29, 1.29±0.37 vs 1.00±0.30, 81.18% vs 40.00%, 68.24% vs 36.47%, respectively, all P<0.05), and the mRNA expression level and high expression rate of E-cadherin were lower than those in adjacent tissues(0.72±0.19 vs 1.00±0.21,23.53% vs 55.29%, all P<0.001). The expression level of lncRNA HOTAIR was negatively correlated with the mRNA expression level and high expression rate of E-cadherin, and positively correlated with the mRNA expression level and high expression rate of N-cadherin and Vimentin (all P<0.05). The expression level of lncRNA HOTAIR in esophageal squamous cell carcinoma with maximum diameter > 4.5 cm, T3 stage and N1~2 stage were higher than those in esophageal squamous cell carcinoma with maximum diameter≤4.5 cm, T1~2 stage and N0 stage(1.71±0.49 vs 1.48±0.44, 1.67±0.44 vs 1.43±0.52, 1.68±0.40 vs 1.44±0.54, all P<0.05). The OS and DFS of patients with lncRNA HOTAIR expression level ≥1.57 (median level) in esophageal squamous cell carcinoma were shorter than those with lncRNA HOTAIR expression level<1.57 (P<0.05). [Conclusion]The increased expression of lncRNA HOTAIR in esophageal squamous cell carcinoma relates to pathological progress and poor prognosis. Promoting EMT may be the molecular mechanism responsible for high expression of lncRNA HOTAIR involved in the pathogenesis of esophageal squamous cell carcinoma.
