      摘 要:[目的]分析食管黏液表皮样癌(mucoepidermoid carcinoma,MEC)患者的临床病理特征,并探讨PD-L1表达对预后的影响。[方法] 收集河北医科大学第四医院2003年1月至2017年12月术后病理确诊为食管MEC患者39例,采用免疫组化染色法测定PD-L1表达,综合阳性分数(CPS)≥10判读为PD-L1阳性。 [结果] 食管MEC患者的 PD-L1阳性率为33.3%(13/39),中位生存期为16个月。PD-L1表达与年龄、性别、临床分期、淋巴结转移等病理特征无关(P>0.05)。Kaplan-Meier单因素分析显示淋巴结转移(χ2=9.541,P=0.002)、临床分期(χ2=5.381,P=0.020)、PD-L1表达(χ2=7.023,P=0.008)对患者预后有影响。Cox多因素分析显示淋巴结转移 (HR=4.510,95%CI:1.669~12.185,P=0.003)、PD-L1表达(HR=3.130,95%CI:1.366~7.172,P=0.007)是食管MEC患者预后的独立影响因素,淋巴结转移、PD-L1阳性的患者预后更差。[结论] PD-L1表达是食管MEC患者预后评估的生物学指标。

      Abstract: [Objective] To investigate the expression of PD-L1 in esophageal mucoepidermoid carcinoma(MEC) and its relation with the prognosis of patients. [Methods] Thirty-nine patients with esophageal MEC who underwent radical surgical resection in the Fourth Hospital of Hebei Medical University from January 2003 to December 2017 were enrolled in the study. The expression of PD-L1 was detected by immunohistochemical stain, the composite positive score(CPS) ≥10 was defined as PD-L1 positive. [Results] The positive rate of PD-L1 was 33.3%(13/39). The median survival time of PD-L1 positive patients was 16 months. There was no significant difference in age, gender, clinical stage, lymph node metastasis between patients with positive and negative PD-L1 expression(P>0.05). Univariate regression analysis showed that lymph node metastasis(χ2=9.541, P=0.002), clinical stage(χ2=5.381, P=0.020) and PD-L1 expression(χ2=7.023, P=0.008)were significantly associated with the prognosis of patients(P<0.05). Cox multivariate regression analysis showed that lymph node metastasis(HR=4.510, 95%CI:1.669~12.185, P=0.003 ) and PD-L1 expression(HR=3.130, 95%CI:1.366~7.172, P=0.007) were independent risk factors for poorer prognosis of patients with esophageal MEC. [Conclusion] The expression of PD-L1 can affect the prognosis of esophageal MEC and may be used as a biological indicator to evaluate the prognosis of patients.
