      摘 要:对于不适合手术的早期非小细胞肺癌患者,立体定向消融放射治疗是首选的治疗手段。近年来,射频消融、微波消融和冷冻消融等热消融治疗也渐见有报道。尽管局部消融治疗展现了良好的局部控制,但总体区域及远处转移率仍较高。因此,结合有效的全身治疗是探索的方向。立体定向消融放射治疗和热消融参与“癌症-免疫周期”,可以促进肿瘤抗原释放、提呈,激活T细胞;也可以重塑肿瘤免疫微环境,将“冷肿瘤”转变为“热肿瘤”。局部消融联合免疫检查点抑制剂不仅能克服消融抵抗、免疫耐药和免疫逃逸,而且能增强远隔效应和免疫记忆效应,从而发挥协同抗肿瘤作用。全文综述了局部消融联合免疫检查点抑制剂的协同作用机制,并探讨治疗早期非小细胞肺癌的潜在疗效。
英文摘要:
      Abstract: Stereotactic ablative radiotherapy(SABR) is the preferred treatment for early non-small cell lung cancer(NSCLC) patients who are not suitable for surgery. In recent years, thermal ablation such as radiofrequency ablation, microwave ablation and cryoablation have been also used for treatment of NSCLC. Although local ablation therapy shows favourable local control, regional and distant metastasis rates are still high. Therefore, a combination of effective systemic therapy is required. SABR and thermal ablation, involved in the “cancer-immune cycle”, can promote the release and presentation of tumor antigens, and activate T cells, also reshape the tumor immune microenvironment from “cold tumors” to “hot tumors”. Local ablation combined with immune checkpoint inhibitors(ICIs) can not only overcome resistance of tumor to the therapy and immune evasion, but also enhance abscopal effect and immunologic memory, exerting synergistic antitumor effects. This article summarizes the synergetic mechanisms of local ablation combined with ICIs, and discusses the potential efficacy in the treatment of early NSCLC.
