      摘 要:[目的] 探讨基于美国癌症联合委员会(AJCC)第八版预后分期标准下不同分期乳腺癌微小RNA-21(microRNA-21,miR-21)、程序性细胞死亡因子4(programmed cell death factor 4,PDCD4)表达差异性,并分析其超声特征。[方法] 选择皖西卫生职业学院附属医院2017年12月至2020年12月经病理学确诊为乳腺癌118例,通过免疫组化和聚合酶链反应检测癌组织和癌旁正常组织中miR-21和PDCD4的表达,分析乳腺癌miR-21和PDCD4表达差异及与超声特征间的关系。[结果] Ⅰ~Ⅲ期乳腺癌miR-21及PDCD4表达水平差异显著,分期越晚,miR-21水平越高,PDCD4水平越低,差异均有统计学意义(P<0.05)。miR-21和PDCD4在乳腺癌中的表达呈负相关(P<0.05)。乳腺癌中miR-21高表达和PDCD4低表达与彩色多普勒超声检测到的边缘毛刺征、微钙化、肿瘤血供分级和淋巴结转移有关(P均<0.05)。[结论] 基于AJCC分期标准下Ⅰ~Ⅲ期乳腺癌miR-21和PDCD4表达水平差异显著,动态监测miR-21和PDCD4的表达水平有助于评估乳腺癌患者个体化治疗效果及预后。对微钙化、边缘毛刺征、肿瘤血供分级和淋巴结转移超声特征进行综合考虑将有助于对乳腺癌患者预后作出初步判断。

      Abstract: [Objective] To investigate the expression of microRNA-21(miR-21) and programmed cell death factor 4(PDCD4) in breast cancer stage Ⅰ~Ⅲ based on the American Joint Commission on cancer(AJCC) 8th Edition prognostic staging criteria, and their relationship with ultrasound features. [Methods] A total of 118 patients with breast cancer staged by AJCC criteria in the Affiliated Hospital of Wanxi Health Vocational College from December 2017 to December 2020 were enrolled in the study. The expression of miR-21 and PDCD4 in cancer tissues and adjacent normal tissues were detected by immunohistochemistry and polymerase chain reaction. The expression differences of miR-21 and PDCD4 in stage Ⅰ~Ⅲ breast cancer and their relationship with ultrasound features were analyzed. [Results] The expression levels of miR-21 and PDCD4 in stage Ⅰ, Ⅱ and Ⅲ breast cancer were significantly different, and the stage was positively correlated with miR-21 expression level and negatively correlated with PDCD4 level(P all <0.05). The expression of miR-21 and PDCD4 in breast cancer was negatively correlated with each other(P<0.05). The color Doppler ultrasonography of patients with high miR-21expression and low PDCD4 expression were likely to present edge spicule sign, microcalcification and high degree blood supply. [Conclusion] The expression levels of miR-21 and PDCD4 in AJCC stage Ⅰ~Ⅲ breast cancer are significantly different. The comprehensive consideration of microcalcification, spicule signn, tumor blood supply grade and lymph node metastasis on ultrasonography will help to make a preliminary judgment of the prognosis of breast cancer patients.
