摘 要:[目的] 研究长链非编码RNA(lcnRNA)HCP5/微小RNA(miR)-27b-3p轴与早期胃癌内镜黏膜下剥离术(ESD)后复发的关系。[方法] 选择468例接受ESD的早期胃癌患者并随访术后复发情况,选择同期105例接受胃镜下胃黏膜活检并经病理证实为正常胃黏膜的受试者作为对照。检测早期胃癌组织及正常胃黏膜组织中lcnRNA HCP5、miR-27b-3p的表达水平,采用Kaplan-Meier曲线分析lcnRNA HCP5、miR-27b-3p与早期胃癌术后复发的关系,采用COX回归模型分析早期胃癌术后复发的影响因素。[结果] 早期胃癌组织中lcnRNA HCP5的表达水平(1.28±0.36)高于正常胃黏膜组织(1.00±0.22),miR-27b-3p表达水平(0.81±0.24)低于正常胃黏膜组织的(1.00±0.27)(P<0.05)。不同肿瘤直径、浸润深度、淋巴结转移、脉管癌栓的早期胃癌组织比较,lcnRNA HCP5、miR-27b-3p表达水平差异有统计学意义(P<0.05)。复发的早期胃癌患者lcnRNA HCP5表达水平(1.67±0.14)高于未复发者(1.27±0.02)(P<0.001),miR-27b-3p表达水平(0.68±0.05)低于未复发者(0.81±0.01)(P=0.020);lcnRNA HCP5高表达患者无复发生存率低于低表达患者(P=0.019),miR-27b-3p高表达患者无复发生存率高于低表达患者(P=0.033)。浸润深度、淋巴结转移、脉管癌栓、lcnRNA HCP5及miR-27b-3p表达是早期胃癌术后复发的影响因素。[结论] 早期胃癌中lcnRNA HCP5/miR-27b-3p轴的变化与多项病理特征恶化及ESD术后复发有关。 |

Abstract:[Objective] To investigate the relationship between long non-coding RNA(lcnRNA) HCP5/microRNA(miR)-27-3p axis and recurrence of early gastric cancer patients after endoscopic submucosal dissection(ESD). [Methods] A total of 468 patients with early gastric cancer who received ESD were enrolled in the study;and 105 subjects who received endoscopic gastric mucosal biopsy and were confirmed as normal gastric mucosa by pathology served as the control group(n=105). The expression levels of lcnRNAHCP5 and miR-27b-3p in early gastric cancer and normal gastric mucosa were detected. The relationship between lcnRNAHCP5,miR-27b-3p and postoperative recurrence of early gastric cancer was analyzed by Kaplan-Meier curve. The influencing factors of postoperative recurrence of early gastric cancer were analyzed by COX regression model. [Results] The expression level of lcnRNAHCP5 in early gastric cancer(1.28±0.36) was higher than that in normal gastric mucosa(1.00±0.22),and the expression level of miR-27b-3p(0.81±0.24) was lower than that in normal gastric mucosa(1.00±0.27)(P<0.05). The expression levels of lcnRNAHCP5 and miR27-3p in early gastric cancer were significantly associated with tumor diameter,invasion depth,lymph node metastasis and vascular tumor thrombus(P<0.05). The expression level of lcnRNAHCP5 in recurrent patients(1.67±0.14) was higher than that in non-recurrent patients(1.27±0.02,P<0.001),and the expression level of miR-27b-3p(0.68±0.05) was lower than that in non-recurrent patients(0.81±0.01,P=0.020). The recurrence-free survival(RFS) rate of patients with high expression of lcnRNAHCP5 was lower than that of patients with low expression of lcnRNAHCP5(P=0.019),and the RFS of patients with high expression of miR-27b-3p was higher than that patients with low expression of miR-27b-3p(P=0.033). Invasion depth,lymph node metastasis,vascular tumor thrombus, lcnRNAHCP5 and miR-27b-3p were the influencing factors of postoperative recurrence of early gastric cancer. [Conclusion] The change of lcnRNAHCP5/miR-27b-3p axis in early gastric cancer is related to the pathological features of early gastric cancer and the recurrence after ESD in the patients. |
