摘 要:[目的] 探讨特异性顶部盘状底板反应蛋白1(Rspo1)与上皮间质转化(epithelial-mesenchymal transition,EMT)相关蛋白波形蛋白(Vimentin)和E-钙黏蛋白(E-cadherin)在胃癌中的表达水平及其临床意义。[方法] 通过免疫组化SP法测定115例胃癌组织及20例癌旁正常组织标本中Rspo1和Vimentin、E-cadherin的表达,分析其与临床病理特征的关系,Rspo1和EMT相关蛋白之间的相关性以及与胃癌患者预后的关系。[结果]胃癌组织中Rspo1和Vimentin阳性表达率(54.8%和34.8%)明显高于癌旁正常组织(25.0%和0)(P均<0.05),而胃癌组织中E-cadherin阳性表达率(53.9%)明显低于癌旁正常组织(100%)(P<0.05)。胃癌组织中Rspo1表达与肿瘤大小、TNM分期、浸润深度、淋巴结转移及远处转移相关(P均<0.05),与患者性别、年龄、分化程度无关。Rspo1与Vimentin的表达呈正相关(r=0.333,P<0.05),与E-cadherin的表达呈负相关(r=-0.349,P<0.05)。Rspo1和Vimentin高表达患者生存率更低(P<0.05),E-cadherin阴性组患者较阳性组患者生存率更低(P<0.05)。多因素Cox回归分析显示:TNM分期Ⅲ~Ⅳ期、淋巴结转移、远处转移、Rspo1表达阳性及E-cadherin表达阴性是影响胃癌患者预后的独立危险因素(P<0.05)。[结论] Rspo1在胃癌组织中高表达,且与EMT相关蛋白的表达密切相关,Rspo1可能通过诱导EMT的发生促进胃癌的侵袭和转移,进而影响患者预后。 |

Abstract:[Objective]To investigate the expression and clinical significance of roof plate specific spondin 1(Rspo1) and epithelial-mesenchymal transition(EMT)-related proteins Vimentin and E-cadherin in gastric cancer. [Methods] Immunohistochemical staining was performed to determine the expression levels of Rspo1,Vimentin and E-cadherin in 115 samples of gastric cancer tissue and 20 samples of paracancerous tissue. Relationship between survival rates and clinicopathological features of the patients with gastric cancer,and correlation of Rspo1 with E-cadherin and Vimentin,were analyzed. [Results] The positive rate of Rspo1 expression and Vimentin in gastric cancer tissue(54.8% and 34.8%) were significantly higher than those in paracancerous tissues(25.0% and 0)(all P<0.05),and the positive rate of E-cadherin in gastric cancer tissue(53.9%) was significantly lower than that in paracancerous tissues(100%)(P<0.05). The high expression of Rspo1 in tumor tissues was closely related to tumor size,TNM stage,depth of invasion,lymph node metastasis,and distant metastasis(all P<0.05),but was not associated with gender,age,and tumor differentiation. The expression of Rspo1 in gastric cancer tissues was negatively correlated with E-cadherin(r=-0.349,P<0.05) and positively correlated with Vimentin(r=0.333,P<0.05). Patients with high expression levels of Rspo1 and Vimentin had significantly poorer prognosis and lower survival rate than those with low expression(P<0.05),whereas patients with low expression levels of E-cadherin had significantly poorer prognosis and lower survival rate than those with high expression(P<0.05). Cox regression analysis showed that TNM stage Ⅲ~Ⅳ stage,lymph node metastasis,distant metastasis,positive Rspo1 expression,and negative E-cadherin expression were the independent prognostic factors of gastric cancer(P<0.05). [Conclusion] Rspo1 is highly expressed in gastric cancer tissues,which is significantly associated with EMT-related proteins(E-cadherin,Vimentin). Rspo1 may promote the invasion and metastasis of gastric cancer by inducing the occurrence of EMT,thereby affecting the prognosis of patients. |
