| 摘 要：胰腺癌是高度恶性、预后极差的肿瘤性疾病，且早期缺乏典型症状。近二十年的大量研究表明，胰腺癌与糖尿病的关系互为因果，糖尿病既是胰腺癌的危险因素，也是其引起的继发性疾病之一。2型糖尿病或高血糖可通过多种机制促进胰腺癌的发生发展，并影响患者的治疗效果和生存结局；而胰腺癌细胞能够以旁分泌或外泌体的方式诱导β细胞功能障碍和胰岛素抵抗，导致新发糖尿病。因此，进一步研究胰腺癌与糖尿病互相促进的机制，有效识别新发糖尿病患者，探索制定基于糖尿病人群的胰腺癌早期筛查策略，有望实现胰腺癌的早期诊断，并改善患者预后。现就胰腺癌与糖尿病之间的双向因果关系研究进展进行综述。 |

| Abstract：Pancreatic cancer is a highly malignant disease with poor prognosis and lack of early symptoms. Amount of studies over the past decades have suggested that there is a reciprocal causation between pancreatic cancer and diabetes. Diabetes is not only a risk factor for pancreatic cancer，but also one of its secondary diseases. Type 2 diabetes or hyperglycemia could promote the tumorigenesis and development of pancreatic cancer via multiple mechanisms. The pancreatic cancer cells are able to induce the β cells dysfunction and insulin resistance by paracrine or exosomes manner，resulting in new-onset diabetes. Thus，studies on the mechanisms of reciprocal promotion of pancreatic cancer and diabetes，identifying new-onset diabetes patients，screening of pancreatic cancer from the new-onset diabetes patients may contribute to early diagnosis of pancreatic cancer and improve the poor prognosis. In this review，the recent clinical and basic researches on the reciprocal causation of pancreatic cancer and diabetes are summarized. |
