      摘 要:[目的] 探讨肿瘤患者肠道屏障功能的临床影响因素。[方法] 分析162例在院肿瘤患者的血浆二胺氧化酶(DAO)、D-乳酸、细菌内毒素水平的数据,结合患者的临床特征,分析其对肠道屏障功能的影响。[结果] 162例患者中,74例(45.68%)DAO值在正常范围内,88例升高(54.32%);89例(54.94%)D-乳酸值在正常范围内,73例升高(45.06%);145例(89.51%)细菌内毒素值在正常范围内,17例升高(10.49%)。单因素分析结果显示,营养不良C级、大便习惯改变、合并肠梗阻、合并感染患者的DAO、细菌内毒素水平显著升高(P均<0.05);大便习惯改变、合并肠梗阻、合并感染患者的D-乳酸水平显著升高(P均<0.05)。多因素Logistic回归分析结果显示,大便习惯改变是导致肿瘤患者血浆D-乳酸水平升高的独立影响因素(OR=11.712,95%CI:4.420~31.034,P<0.05),感染是导致肿瘤患者血浆细菌内毒素水平升高的独立影响因素(OR=3.280,95%CI:1.009~10.664,P<0.05)。[结论] 大便习惯改变、感染分别为影响肿瘤患者血浆D-乳酸、细菌内毒素水平的独立影响因素。

      Abstract:[Objective] To explore the clinical influencing factors of intestinal barrier function in cancer patients. [Methods] One hundred and sixty-two cancer patients were enrolled. Serum levels of Diamine oxidase(DAO),D-lactic acid and bacterial endotoxin were measured,and influencing factors of intestinal barrier function were analyzed. [Results] In 162 patients,DAO levels were increased in 88 cases(54.32%),D-lactic acid levels were increased in 73(45.06%) and bacterial endotoxin levels were increased in 17(10.49%). Univariate analysis showed that the levels of serum DAO and bacterial endotoxin were significantly elevated in patients with grade C malnutrition,change of defecation habit,intestinal obstruction and infection(all P<0.05),while the levels of serum D-lactic acid were significantly elevated in patients with change of defecation habit,intestinal obstruction and infection(all P<0.05). The multivariate Logistic regression analysis showed that change of defecation habit was an independent influencing factor for the high levels of D-lactic acid(OR=11.712,95%CI:4.420~31.034,P<0.05),while infection was an independent influencing factor for the high levels of bacterial endotoxin in cancer patients(OR=3.280,95%CI:1.009~10.664,P<0.05). [Conclusion] Change of defecation habit and infection are the independent influencing factors for the levels of D-lactic acid and bacterial endotoxin,respectively.
