摘 要:[目的] 研究检测宫颈癌尾型同源盒基因2(caudal-related homeobox 2,CDX2)基因启动子区域甲基化程度的临床意义。[方法] 选择50例诊断为宫颈癌患者(观察组)和50例宫颈良性病变患者(对照组),采用甲基化特异性聚合酶链式反应检测组织中CDX2启动子甲基化水平,荧光定量聚合酶链式反应法检测CDX2 mRNA的表达量。根据TNM分期选择治疗方案,记录无瘤生存期、复发率和死亡率。[结果] 观察组的甲基化率为70.0%(35/50),对照组为20.0%(10/50) (P<0.05)。观察组中随肿瘤淋巴结转移分期增加,组织学分级越低及病灶越大,甲基化越高(均P<0.05)。观察组CDX2 mRNA明显低于对照组(P<0.05)。观察组中随TNM分期增加、组织学分级越低及病灶越大,CDX2 mRNA表达越低(P<0.05)。观察组甲基化阳性者无瘤生存期缩短,复发率和死亡率增高。临床分期、组织学分级、病灶大小、CDX2 mRNA和甲基化是宫颈癌生存的独立危险因素(P<0.05)。 [结论] 宫颈癌CDX2基因启动子区域甲基化增加,CDX2蛋白表达降低,导致患者无瘤生存期缩短,复发率和死亡率增高。 |

Abstract:[Objective] To investigate the methylation status of caudal-related homeobox 2(CDX2) gene promoter region in cervical carcinoma and its clinical significance. [Methods] Fifty patients with cervical carcinoma(study group) and 50 patients with cervical benign lesions(control group) were enrolled. The promoter region methylation level of CDX2 gene was detected with methylation specific polymerase chain reaction(MSP),and the expression of CDX2 mRNA was detected by RT-PCR. The disease-free survival(DFS),recurrence and mortality rates were observed in cervical cancer patients. [Results] The methylation rate of the study group was significantly higher than that of the control group(70% vs. 20.0%,P<0.05). The expression of CDX2 mRNA in the study group was significantly lower than that in the control group. The level of CDX2 mRNA expression in the study group was positively correlated with the TNM stage and negatively correlated with the histological grade(P<0.05). The recurrence rate and mortality rate in study group were significantly higher,and DFS was shorter than those in the control group(P<0.05). The clinical stage,histological grade,lesion size,CDX2 mRNA and methylation status of CDX2 were independent risk factors for survival of cervical cancer patients(P<0.05). [Conclusion ] Methylation of CDX2 gene promoter region is increased and CDX2 expression is decreased in cervical cancer,which are associated with the shortened tumor-free survival and the increased recurrence rate and mortality. |
