| 摘 要：［目的］分析伴有肠上皮化生的不同胃黏膜病变中环氧化酶-2(cyclooxygenase-2，COX-2)与真核细胞核因子p65(nuclear factor-kappa B p65，NF-κB p65)表达及与幽门螺杆菌(Helicobacterpylori，Hp)的关系。［方法］收集胃黏膜活检标本或经胃癌手术获得的伴肠化生标本，根据病变程度分为4组：慢性萎缩性胃炎组43例，胃黏膜中、重度异型增生组40例，早期胃癌组37例和进展期胃癌组32例，收集并检测所有胃黏膜组织的HE染色及COX-2、NF-κB p65及Hp表达情况。［结果］ 慢性萎缩性胃炎组、胃黏膜中重度异型增生组、早期胃癌组和进展期胃癌组的COX-2的阳性率分别为11.6%、30.0%、54.1%和78.1%（χ2=38.233，P<0.001），NF-κB p65阳性率分别为16.3%、35.0%、59.5%和84.4%（χ2=38.903，P<0.001），Hp阳性率分别为9.3%、27.5%、51.4%和75.0%（χ2=38.234，P<0.001）。COX-2、NF-κB p65蛋白表达均与Hp呈正相关 (r=0.675，P=0.035；r=0.649，P=0.0319)。［结论］伴有肠上皮化生的不同胃黏膜病变的胃黏膜COX-2与NF-κB p65及Hp均有不同程度地表达，随着胃黏膜病变的加重，胃黏膜中COX-2与NF-κB p65及Hp表达增高。 |

| Abstract：［Objective］ To investigate the expression of cyclooxygenase-2(COX-2) and nuclear factor-kappa B p65(NF-κB p65) protein in different gastric mucosal lesions and relation to Helicobacter pylori(Hp) infection. ［Methods］ The biopsy specimens of gastric mucosa were collected from 43 cases of chronic atrophic gastritis(group A)，40 cases of moderate and severe dysplasia(group B)，37 cases of early gastric cancer(group C) and 32 cases of advanced gastric cancer(group D). The expressions of COX-2 and NF-κB p65 in gastric mucosa was detected with immunohistochemistry and Western blot. Hp infection was determined by 14C breath test. ［Results］ The positive expression of COX-2 in groups A，B，C and D was 11.6%，30.0%，54.1% and 78.1%，respectively(χ2=38.233，P<0.001)；the positive expression of NF-κB p65 in 4 groups was 16.3%，35.0%，59.5% and 84.4%，respectively(χ2=38.903，P<0.001)；and the positive Hp infection rates in 4 groups were 9.3%，27.5%，51.4% and 75.0%，respectively(χ2=38.234，P<0.001). The expression of COX-2 and NF-κB p65 protein was positively correlated with Hp infection(r=0.675，P=0.035；r=0.649，P=0.0319). ［Conclusion］ The positive expression rate of COX-2，NF-κB p65 and Hp infection rate are increased with the aggravation of gastric mucosal lesions. |
