摘 要:[目的] 探讨高频超声评估乳腺癌新辅助化疗疗效的价值。[方法] 选取128例乳腺癌患者均予以新辅助化疗。观察患者新辅助化疗的治疗效果,比较病理有效与无效患者化疗前后的病灶面积、灌注区域,观察术前超声测量肿瘤大小的准确性,比较病理有效与无效患者的最大径(D)、动脉血流阻力指数(RI)、肿瘤内动脉收缩期峰值血流速度(PSV)、弹性应变率比值(ESR)和血流灌注情况。 [结果] 128例患者中,106例病理评价结果为有效,22例为无效。病理有效患者化疗后的病灶面积为(4.41±1.05)cm2,灌注区域为(4.71±1.18)cm2,低于化疗前及无效患者(P<0.05)。术前超声测量肿瘤直径结果共113例与术后病理测量结果保持一致,准确率达88.28%,具有良好一致性(K=0.218)。病理有效患者化疗后的D、RI、PSV、ESR均较化疗前显著性下降(P<0.05),同时低于无效患者(P<0.05)。病理有效与无效患者化疗前的血流分级无统计学意义(P>0.05)。化疗后病理有效患者的整体血流分级分布显著性低于无效患者(P<0.05)。[结论] 高频超声可准确测量肿瘤大小变化并反映肿瘤血流情况,可对乳腺癌新辅助化疗疗效进行定量评估,值得推广应用。 |

Abstract:[Objective] To evaluate the application of high frequency ultrasound in assessing efficacy of neoadjuvant chemotherapy for patients with breast cancer. [Methods] One hundred and twenty eight patients with breast cancer received neoadjuvant chemotherapy,among whom 106 cases were pathologically effective(effective group) and 22 cases were pathologically ineffective(ineffective group). All patients underwent high frequency ultrasonography examination before and after treatment. The lesion area,perfusion area,maximum diameter(D),arterial blood flow resistance index(RI),peak systolic velocity(PSV),elastic strain ratio(ESR),blood perfusion were compared in two groups. [Results] The lesion area and perfusion area of effective group after chemotherapy were(4.41±1.05)cm2 and(4.71±1.18)cm2,which were lower than those before chemotherapy and also lower than those in ineffective patients(P<0.05). Compared to postoperative pathological findings,the accuracy of preoperative tumor diameter measured by ultrasound was 88.28%(113/128) with a good consistency(K=0.218). The levels of D,RI,PSV and ESR in effective group after chemotherapy were decreased than those before chemotherapy(P<0.05),and also lower than those of ineffective group(P<0.05). There was no significant difference in blood flow grade before chemotherapy between two groups(P>0.05),while the overall distribution of blood flow in effective group was significantly lower than that in ineffective group after chemotherapy(P<0.05). [Conclusion] High frequency ultrasound can accurately measure the changes of tumor size and reflect the tumor blood flow,which can be used for quantitative evaluation of efficacy of neoadjuvant chemotherapy in patients with breast cancer. |
