| 摘 要：［目的］分析不同方法检测口咽鳞癌中高危型HPV在河南地区人群中的感染情况。［方法］ 采用二代杂交捕获、p16免疫组化、bDNA三种方法对89例口咽鳞癌石蜡包埋标本进行检测，并分析其HPV阳性率与患者临床病理参数的相关性及其临床意义。［结果］ 二代杂交捕获、p16免疫组化、bDNA三种检测方法结果具有较好的一致性，HPV阳性率分别为27.0%，22.5%，16.9%。三种方法检出的HPV阳性结果与患者性别、年龄、原发灶部位、肿瘤分化程度、T分期等临床病理参数均无关（P＞0.05）。经p16免疫组化法检测的结果与患者淋巴结转移（P=0.049）、TNM分期（P=0.026）存在相关性，而二代杂交捕获、bDNA结果与患者淋巴结转移、TNM分期（P＞0.05）均无关。［结论］ 在充分考虑临床实用性及可靠性的前提下，p16蛋白免疫组化法是最简单实用的HPV感染检测方法。 |

| Abstract：［Objective］ To investigate the infection of high-risk HPV in oropharyngeal squamous cell carcinoma by a variety of methods. ［Methods］ The hybrid capture-Ⅱ，p16 immunohistochemistry and bDNA methods were used to detect paraffin- embedded specimens from 89 patients with oropharyngeal squamous cell carcinoma. The correlation between the positive rate of HPV and clinicopathological parameters and its clinical significance were analyzed. ［Results］ The three detection methods showed a good consistency，and the positive rates of detection were 27%，22.5% and 16.9% respectively. The HPV results obtained from the three detection methods were not related to the gender，age，primary tumor location，tumor differentiation and T staging(P>0.05). The results of p16 immunohistochemistry were correlated with lymph node metastasis(P=0.049) and TNM staging(P=0.026)，while the results of hybrid capture-Ⅱ and bDNA were not related to the lymph node metastasis and TNM staging(P>0.05). ［Conclusion］ Compared to other methods，p16 protein immunohistochemistry is the simplest and most effective method for high-risk HPV infection detection in patients with oropharyngeal squamous cell carcinoma. |
