| 摘 要：［目的］ 探索AFP和PIVKA-Ⅱ对PHC诊断的临床运用。［方法］ 收集原发性肝细胞癌87例、肝硬化39例、慢性肝病39例和体检健康对照25例的病例资料，对各组检测血清AFP和PIVKA-Ⅱ水平。［结果］ 与健康对照组相比，各肝病组AFP和PIVKA-Ⅱ均有不同程度升高（P<0.01）；PHC组AFP和PIVKA-Ⅱ水平高于慢性肝病组和肝硬化组（P<0.01）。 AFP和PIVKA-Ⅱ在辅助诊断PHC的敏感度和特异性分别为72%、68%和79%、62%；两者联合诊断时敏感度和特异性分别为91%和85%。［结论］ AFP和PIVKA-Ⅱ联合诊断时可以进一步提高诊断的敏感度或特异性。 |

| Abstract：［Objective］ To evaluate the value of AFP and PIVKA-Ⅱ for diagnosing primary hepatocellular carcinoma(PHC). ［Methods］ The 87 samples of PHC，39 samples of hepatocirrhosis，39 samples of chronic hepatitis，and 25 samples of health adults were tested AFP by ECLIA，and PIVKA-Ⅱ by CLEIA simultaneously. ［Results］ AFP and PIVKA-Ⅱ of hepatopathy groups were higher than those in health group(P<0.01). AFP and PIVKA-Ⅱ of PHC group were highest among that of hepatocirrhosis and chronic hepatitis groups(P<0.01). The sensitivity and specificity of AFP and PIVKA-Ⅱ for hepatocellular carcinoma were 72%，68% and 79%，62% respectively. And the sensitivity and specificity of AFP combining with PIVKA-Ⅱwas 91%，85%. ［Conclusion］ AFP and PIVKA-Ⅱ are effective markers to examine PHC. The sensitivity or specificity of combining with AFP，PIVKA-Ⅱ would be up for diagnosing PHC，and could reduce the rate of missed diagnosis in PHC of negative AFP especially. |
