      摘 要:[目的] 探讨18F-FDG PET/CT显像在不明原因发热患者(fever of unknown origin,FUO)中筛查淋巴瘤的价值。[方法] 回顾18F-FDG PET/CT显像考虑为淋巴瘤可能的36例FUO患者资料,分析比较18F-FDG PET/CT检查结果与最终诊断。同时记录患者基本信息包括年龄、性别及发热持续时间;体格检查包括最高体温及热度;实验室检查包括白细胞、中性粒百分比、血小板、血红蛋白、ESR、CRP、PCT等;组间差异比较采用t检验或卡方检验。[结果] 36例18F-FDG PET/CT考虑为淋巴瘤可能的患者中,最终病理明确为淋巴瘤者20例,占55.6%(20/36)。与非淋巴瘤组相比,淋巴瘤患者SUVmax显著升高(9.02±4.03 vs 4.94±2.71,t=3.466,P=0.001)。根据患者最高体温分为中度热、高热及超高热,组间发热程度差异有统计学意义(χ2=8.23,P=0.016)。与淋巴瘤组患者相比,非淋巴瘤组患者发热持续时间更长。而年龄、性别、伴随症状、体征及实验室检查在两组间差异无统计学意义(P均>0.05)。[结论] 18F-FDG PET/CT显像对淋巴瘤有一定的诊断价值。病灶SUVmax值、患者发热程度及发热持续时间对FUO中淋巴瘤的筛查提供重要参考。

      Abstract:[Objective] To evaluate the application of 18F-FDG PET/CT for screening of lymphoma in patients with fever of unknown origin(FUO). [Methods] Clinical data of 36 patients with FUO undergoing 18F-FDG PET/CT examination were retrospectively reviewed. The results were compared with final diagnosis based on the biopsy,surgical pathology or clinical follow-up. Basic information of patients including age,gender,degree and duration of fever,physical examination,white blood count,percentage of neutrophils,platelet count,hemoglobin,ESR,CRP and PCT were recorded. [Results] Among 36 FUO patients,20(55.6%) were diagnosis as lymphoma by 18F-FDG PET/CT. Compared with the non-lymphoma patients,SUVmax was higher in lymphoma patients(9.02±4.03 vs 4.94±2.71,t=3.466,P=0.001). Non-lymphoma patients had a longer duration of fever. [Conclusion] 18F-FDG PET/CT is a valuable imaging technique for screening lymphoma in FUO patients,SUVmax,degree and duration of fever may provide important information for screening.
