      摘 要:[目的] 分析不同分子亚型乳腺癌的全数字化X线摄影(FFDM)表现,探讨乳腺癌分子亚型与FFDM表现的关系。[方法] 对200例乳腺癌患者术前行乳腺FFDM检查,术后采用免疫组化检测,依据ER、PR、HER-2表达情况分为4种分子亚型,观察不同分子亚型乳腺癌的FFDM表现,并分析乳腺癌分子亚型与FFDM表现的关系。[结果] 200例乳腺癌中单纯肿块型83例(41.5%),单纯钙化型39例(19.5%),肿块伴钙化型64例(32.0%),局部密度增浓型14例(7.0%)。Luminal A型78例(39.0%),Luminal B型54例(27.0%),HER-2型43例(21.5%),Basal-like型25例(12.5%)。Luminal A型和Luminal B型主要表现为单纯肿块型(57.7%和51.9%),HER-2型主要表现为单纯钙化型(48.8%),Basal-like型主要表现为肿块伴钙化型(40.0%)。统计学结果显示,不同分子亚型乳腺癌的FFDM表现差异有统计学意义(P<0.05)。[结论] 乳腺癌的分子亚型与FFDM表现之间有一定的相关性。

      Abstract:[Objective] To study the imaging findings of full-field digital mammography (FFDM) in patients with breast cancer of different molecular subtypes,and to explore the correlation between molecular subtypes with imaging manifestations of FFDM. [Methods] FFDM and immunohistochemistry were given to 200 patients with breast cancer before and after operation respectively. Molecular subtypes were defined by immunohistochemical markers as estrogen receptor (ER),progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER-2). Imaging features of FFDM were observed in different molecular subtypes and correlations were analyzed between molecular subtypes and imaging findings of FFDM. [Results] There were 83 cases (41.5%) with only mass,39 cases (19.5%) with only calcification,64 cases (32.0%) with mass with calcification,14 cases (7.0%) with local density thickener,and 78 cases (39.0%) with Luminal A,54 cases (27.0%) with Luminal B,43 cases(21.5%) with HER-2,25 cases (12.5%) with Basal-like in 200 cases with breast cancer. Luminal A and Luminal B mainly were only mass (57.7%,51.9%). HER-2 mainly was only calcification mass with calcification(48.8%),and Basal-like mainly was local density thickener (40.0%). Statistical analysis showed there were statistical significance in imaging findings of FFDM and different molecular subtypes of breast cancer(P<0.05). [Conclusions] Different molecular subtypes have correlation to imaging findings of FFDM in breast cancer.
