| 摘 要：［目的］ 观察右美托咪定对乳腺癌根治术患者Th1/Th2细胞因子的影响。［方法］ 择期行乳腺癌根治术患者60例，采用随机数字法，分为对照组和右美托咪定组。右美托咪定组予以右美托咪定1?滋g/kg负荷剂量，随后以0.5?滋g/kg·h静脉输注右美托咪定至术毕。对照组予以等量生理盐水。分别于给药麻醉诱导前（T0）、手术结束时（T1）、手术结束1h（T2）、手术结束24h（T3）和手术结束72h（T4）抽取外周静脉血样，采用ELISA法测定血浆IFN-γ、IL-2、IL-4和IL-10浓度，计算IFN-γ/IL-4。［结果］ T0时点，对照组和右美托咪定组各指标无显著差异。与对照组相比，右美托咪定组T1、T2、T3、T4时点的IL-2、IFN-γ、IFN-γ/IL-4升高，IL-10降低，而各时点IL-4差异无统计学意义。［结论］ 右美托咪定可调节机体细胞免疫功能，改善Th1/Th2细胞平衡。 |

| Abstract：［Purpose］ To investigate the effect of dexmedetomidine on the ratio of Th1/Th2 cytokines in patients undergoing breast cancer surgery.［Methods］ Sixty patients scheduled for breast cancer surgery were enrolled.Patients were randomly divided into two groups：dexmedetomidine group(n=30)and control group (n=30).The dexmedetomidine group was infused with a 1.0?滋g/kg loading dose following by infusion of 0.5?滋g/kg·h. The equal volume of saline was given in control group. Venous blood samples were obtained before induction. Plasma concentration of IFN-γ，IL-2，IL-4 and IL-10 at the end of operation and 1，24，72h after operation were detected. The IFN-γ/IL-4 ratio was calculated. ［Results］ There was no significant difference between the two groups before induction（P>0.05）.Compared with control group，the concentration of IL-2，IFN-γ，the ratio of IFN-γ/IL-4 were significantly increased at the end of operation and 1，24，72h after operation in dexmedetomidine group(P<0.05)，while IL-10 decreased. There was no significant change of IL-4 at each time point(P>0.05).［Conclusion］ Dexmedetomidine can improve the celluar immune function，shifting the Th1/Th2 cytokines balance toward Th1 in patients undergoing breast cancer surgery. |
