| 摘 要：［目的］ 探讨HIF-1α、VEGF和pVHL在肾透明细胞癌（clear cell renal cell carcinoma，CCRCC）中的表达水平及与临床病理特征的关系。［方法］ 采用免疫组化法检测55例CCRCC中HIF-1α、VEGF和pVHL 3种蛋白的表达，并对其与临床分期、病理分级、淋巴结转移及预后关系作相关性分析。［结果］ 52例获得随访，5年内共有12例死亡。HIF-1α和VEGF在55例CCRCC中阳性表达率分别为58.2%，100.0%，pT3/pT4期组强表达率（72.7%、54.5%）显著高于pT1/pT2期组（29.5%、20.5%）（Ｐ<0.05)；高级别组（75.0%、41.7%）显著高于低级别组（27.9%、23.3%）(Ｐ<0.05)；两者高表达与淋巴结转移及中位生存时间密切相关（Ｐ<0.05），且两者表达存在正相关关系（r=0.37，Ｐ<0.05)。pVHL阳性表达率为52.7%（29/55），与病理分级、临床分期、淋巴结转移及HIF-1α均无明显相关性（P>0.05）。［结论］ HIF-1α和VEGF在肾透明细胞癌中的表达可作为预测患者预后的指标，共同强表达预示生存时间短，对临床早期干预治疗具有重要的指导意义。 |

| Abstract：［Purpose］ To investigate expressions of HIF-1α，VEGF and pVHLin clear cell renal carcinoma (CCRCC) and their clinical significance. ［Methods］ The protein expressions of HIF-1α，VEGF and pVHL in CCRCC were evaluated immunohistochemically and the correlation among their expressions，clinical stage，pathological grade，lymph node metastasis and prognosis was analyzed.［Results］ Fifty-two cases were followed up，and 12 cases died in 5 years. The expressions of HIF-1α and VEGF in 55 patients with CCRCC were 58.2% and 100.0% respectively，and their expressions in stage pT3/pT4 group(72.7% and 54.5%) were significantly higher than those in stage pT1/pT2 group(29.5% and 20.5%)(P<0.05)；and their expressions in high grade patients (75.0% and 41.7%) were higher than those in low grade patients (27.9% and 23.3%) (P <0.05).High expressions of HIF-1α and VEGF significantly correlated with lymph node metastasis and median survival(P<0.05)，and a positive correlation between HIF-1α and VEGF was found(r=0.37，P<0.05). Positive expression of pVHL was 52.7%（29/55） in CCRCC patients，which had no correlation with pathological grade，or clinical staging，or lymph node metastasis，or HIF-1α(P>0.05).［Conclusion］ HIF-1α and VEGF expression might be biomarker for assessment of CCRCC prognosis. Their strong coexpression in CCRCC predicts short survival，which plays an important significance for early intervention treatment of CCRCC. |
