| 摘 要：［目的］评价引起甲状腺功能亢进症自主功能性甲状腺瘤及弥漫肿大131I治疗的消融作用。［方法］182例甲亢患者，于131I治疗前进行甲状腺显像，诊断甲亢病因；131I治疗后6个月再次进行甲状腺显像，观察比较治疗前后131I的消融结果。［结果］ 182例患者中甲状腺自主性高功能腺瘤18例（9.89%），多结节性甲状腺肿伴甲亢5例（2.75%）；弥漫性甲状腺肿大伴甲亢（Graves’病）159例（87.36%）。甲状腺自主性高功能腺瘤18例患者均治愈，有2例出现甲减。Graves’病一次治愈130例（81.8％），其中甲状腺功能正常99例（62.3％），早发甲减31例（19.5％）；未愈29例（18.2%）。治疗前和131I消融后6个月甲状腺显像甲状腺质量明显缩小，分别为49.1g和26.9g。［结论］ 131I对甲状腺瘤及弥漫肿大有显著消融作用。 |

| Abstract：［Purpose］ To investigate the effect of 131I ablation for high functional thyroid nodular and diffuse goiter. ［Methods］ A total of 182 patients with hyperthyroidism undergoing thyroidual nuclide imaging pre radioiodine-131 therapy，and divided into 3 types：Graves’ disease，thyroid toxic adenoma and thyroid toxic multinodular goiter. And the results pre and post 131I ablation were compared.［Results］Of 182 cases，thyroid autonomy high functional adenoma was 18(9.89%) cases；toxic multinodular goiter，5(2.75%) cases；Graves’ disease，159(87.36%). All 18 cases with thyroid autonomous functioning adenomas were cured，while 2 patients occurred hypothyroidism. One hundred and thirty cases with Graves’ disease were cured，accounted for 81.8%，99 patients(62.3%) had normal thyroid function，31 patients(19.5%) occurred early hypothyroidism，and 29 patients(18.2%) were not cured. Compared with thyroid weight before iodine-131 treatment (49.1g)，it reduced after 6 months iodine-131 treatment (26.9g).［Conclusions］ Radioiodine-131 has significant ablation effect for thyroid adenoma and diffuse goiter. |
