| 摘 要：［目的］ 探讨三维适形放疗同步化疗治疗直肠癌术后复发的疗效。［方法］ 78例术后复发直肠癌患者分为适形放疗加化疗组（适形组）39例，常规放疗加化疗组（常规组）39例。观察并比较两组的临床疗效和不良反应的发生。［结果］ 适形组和常规组1、2、3年生存率分别为89.7％、64.1％、46.2％和64.1％、35.9％、23.1％(P<0.05)，中位生存期分别为35个月和20个月；3年肿瘤局部控制率分别为64.1％、41.0％（P<0.05）；适形组放射性直肠炎、放射性膀胱炎和皮肤反应的发生率低于常规组（P<0.05）。［结论］ 三维适形放射治疗联合FOLFOX方案同步化疗是治疗直肠癌术后复发的安全有效的方法，能明显地提高患者的远期生存率，减少放疗反应。 |

| Abstract: ［Purpose］ To investigate the clinical efficacy of concurrent three-dimensional conformal radiotherapy and chemotherapy(3D-CRC) in the treatment for rectal cancer with local recurrence postoperation.［Methods］ Seventy-eight rectal cancer patients with local recurrence postope-ration were divided into two groups: 3D-CRC group (39 cases) and conventional radiochemotherapy(RC) group (39 cases).The clinical efficacy and side effects were observed and compared. ［Results］ The 1-，2- and 3- year survival rates were 89.7％，64.1％，46.2％ in 3D-CRC group，64.1％，35.9％，23.1％ in RC group，with significant difference(P<0.05). The median survival time in the 2 groups was 35 months and 20 months respectively. The 3-year local control rate of 3D-CRC group and RC group were 64.1％and 41.0％,respectively (P<0.05). The incidences of radiation proctitis，radiocystitis and skin reaction were lower in 3D-CRC group than those in RC group（P<0.05）. ［Conclusion］ Concurrent three-dimensional conformal radiotherapy and chemotherapy might be safe and effective in the treatment for rectal cancer with local recurrence postoperation，and may improve survival rate with less toxicity. |
