      肿瘤相关巨噬细胞与铁死亡作为参与构建肿瘤免疫抑制微环境、调控消化系统肿瘤恶性进展的核心关键因素,其具体作用机制及临床转化价值已成为当前研究热点。本文通过系统综述TAMs与铁死亡在消化系统肿瘤(胃癌、肝细胞癌、结直肠癌等)及炎症性癌前病变(幽门螺旋杆菌相关性胃炎、溃疡性结肠炎、肝纤维化等)中的双向交互调控机制进行阐述及总结,同时梳理了针对TAMs与铁死亡的新型治疗手段,以及进一步讨论二者在消化系统肿瘤中作用机制的研究现状、新型治疗手段的临床应用进展及现存不足,旨在为消化系统肿瘤的精准治疗提供理论参考。

      Tumor-associated macrophages (TAMs) and ferroptosis are recognized as pivotal regulators in shaping the immunosuppressive tumor microenvironment (TME) and driving the malignant progression of digestive system malignancies. Elucidating their underlying molecular mechanisms and evaluating their translational potential have become prominent areas of contemporary oncological research. In this review, we systematically delineate the bidirectional crosstalk between TAMs and ferroptosis in digestive system cancers, including gastric cancer, hepatocellular carcinoma (HCC), and colorectal cancer (CRC). In addition, we extend the discussion to inflammation-associated premalignant conditions, such as Helicobacter pylori–associated gastritis, ulcerative colitis (UC), and hepatic fibrosis, highlighting their roles in tumor initiation and progression. Moreover, we comprehensively summarize emerging therapeutic strategies targeting TAMs and ferroptosis, and critically evaluate recent advances in their clinical translation. We also address current challenges and limitations in this field. Collectively, this review aims to provide a theoretical framework and potential directions for the development of precision medicine approaches in digestive system malignancies.
