      仑伐替尼联合免疫检查点抑制剂(immune checkpoint inhibitors,ICIs)已成为不可切除或中晚期肝癌系统治疗和转化治疗的重要策略之一。肿瘤免疫微环境重塑是其协同抗肝癌的核心机制,目前该领域机制解析与临床转化仍有诸多研究新进展待整合。本文围绕肝癌肿瘤免疫微环境(tumor immune microenvironment,TIME)免疫抑制形成机制,系统综述仑伐替尼联合ICIs对肝癌TIME的动态重塑效应,包括促进肿瘤血管正常化、缓解组织缺氧、逆转代谢重编程以及免疫细胞群落的动态重排。同时系统分析了多项关键临床试验的疗效数据,探讨当前联合疗法在实际应用中的优势以及局限性,为肝癌的个体化精准防控与基础研究提供参考。

      Lenvatinib combined with immune checkpoint inhibitors (ICIs) has emerged as one of the pivotal systemic and conversion therapeutic strategies for unresectable, intermediate and advanced hepatocellular carcinoma (HCC). Remodeling of the tumor immune microenvironment (TIME) constitutes the core mechanism underlying their synergistic anti-HCC activity, and numerous novel research advances regarding mechanistic elucidation and clinical translation in this field remain to be comprehensively summarized. Focusing on the drivers of immunosuppressive TIME formation in HCC, this paper systematically reviews the dynamic remodeling effects exerted by lenvatinib plus ICIs on HCC TIME, including facilitating tumor vascular normalization, alleviating tissue hypoxia, reversing metabolic reprogramming, and dynamically rearranging immune cell populations. Meanwhile, efficacy data from multiple pivotal clinical trials are systematically analyzed to discuss the merits and limitations of this combination regimen in real-world clinical practice, thereby providing evidence-based references for individualized precise prevention and treatment as well as basic translational research of HCC.
