      目的:探讨碳离子治疗(CIRT)乳腺癌的疗效与安全性。方法:回顾性收集甘肃省武威肿瘤医院接受碳离子治疗并完成随访的20例乳腺癌患者临床资料,根据肿瘤大小、肿瘤距离皮肤距离、肿瘤类型等因素,在保证皮肤能耐受的前提下,给予肿瘤72-80Gy (RBE)的处方剂量,阳性淋巴结给予56-60Gy (RBE),全乳及淋巴引流区给予48Gy (RBE)。结果:根据入选、排除标准共纳入20例患者,中位随访 23.5 个月,不良反应以 1 级急性皮肤反应为主(14 例),仅 1 例出现 2 级皮肤反应、1 例 2 级放射性食管炎,无其他严重不良反应;至末次随访,10例完全缓解,6例部分缓解,4例病情稳定,无疾病进展;患者 1 年和 2 年的局部控制率(LCR)、无进展生存率(PFS)及总生存率(OS)均达到 100%;所有病例的美容效果均获评优异。结论:碳离子放射治疗乳腺癌疗效确切,不良反应轻微可控,为不宜或不愿手术的患者提供了安全有效的非手术治疗选择。
英文摘要:
      Objective: To investigate the efficacy and safety of carbon ion therapy (CIRT) in breast cancer. Methods: The clinical data of 20 breast cancer patients who received carbon ion therapy and completed follow-up in Wuwei Cancer Hospital of Gansu Province were retrospectively collected, and the prescribed dose of 72-80 Gy (RBE) was given to the tumors, 56-60 Gy (RBE) for positive lymph nodes, and 48 Gy (RBE) for whole breast and lymphatic drainage area according to tumor size, distance from the skin, tumor type and other factors. Results: A total of 20 patients were included according to the inclusion and exclusion criteria, with a median follow-up of 23.5 months, and the adverse reactions were mainly grade 1 acute skin reactions (14 cases), only 1 case had grade 2 skin reactions, 1 case had grade 2 radiation esophagitis, and had no other serious adverse reactions. By the last follow-up, 10 cases had complete remission, 6 cases had partial remission, and 4 cases had stable disease without disease progression. Survival analysis showed that the local control rate (LCR), progression-free survival rate (PFS) and overall survival (OS) at 1 and 2 years were all 100%. All cases were rated as excellent in cosmetic results. Conclusion: Carbon ion radiotherapy has a definite efficacy and mild controllable adverse reactions, providing a safe and effective non-surgical treatment option for patients who are not suitable or unwilling to undergo surgery.
