[Purpose] To analyze the compliance and influencing factors of endoscopic screening among high-risk population of upper gastrointestinal cancer (UGC) in Chongqing, in order to provide evidence and suggestions for optimizing the management of UGC screening programs. [Methods] Epidemiological investigation and risk assessment of UGC were conducted among 40-74 years old residents willingly, who were in the areas covered by the Chongqing Urban Cancer Early Diagnosis and Treatment Program from 2012 to 2019. The residents who were assessed as at high risk of UGC were advised to receive endoscopic screening for free in designated hospitals. The compliance and influencing factors of endoscopic screening among high-risk population of UGC were analyzed. [Results] There were 266611 residents who completed the questionnaires and UGC risk assessment. The 48000 residents were assessed as the individuals with UGC high risk, the high risk rate of the UGC was 18.00%. The 9162 residents received the following endoscopic screening, the compliance( screening rate) was 19.09%. The results of unconditional multivariate logistic regression analysis showed that the high-risk participants with following characteristics were more likely to have a high compliance to endoscopic screening: 45-64 years old, with the education of high school/vocational school/technical school or above, divorced/widowed, having occupational exposure to hazardous substances, having hot food preference in daily diet, having high fat content in daily diet, having a frequent consumption of pickled and sun dried food, being exposed to a lot of kitchen fume when cooking at home, suffering from psychic trauma or depression, with upper gastrointestinal disease history, with family history of UGC. While the high-risk participants who aged over 70 years, were current smokers and regularly participated in physical exercise were more likely to have low compliance to endoscopic screening. [Conclusion] Among high-risk population of UGC in Chongqing, the compliance to endoscopic screening still needs be improved. Health education and management should be targeted at different populations with different characteristics to improve overall compliance with endoscopic screening, thereby to enhance the effectiveness of UGC screening and truly to achieve early diagnosis and treatment of UGC. |
