| 摘 要:[目的] 分析广东省 “两癌” (乳腺癌、宫颈癌)筛查项目省级统筹管理地区(简称“省项目地区”)与非省级统筹管理地区(简称“非省项目地区”)乳腺癌筛查现状,揭示不同筛查管理模式对乳腺癌筛查效果的影响。[方法] 基于广东省 21 个地市 2021—2023 年妇女乳腺癌筛查情况年报表,按筛查管理模式将地区分为省项目地区和非省项目地区,采用χ2检验比较不同筛查区域的35~64 周岁妇女乳腺癌人群筛查率、活检遵从率、乳腺癌检出率及早诊率等核心指标。[结果] 2021—2023 年广东省适龄妇女乳腺癌人群筛查率呈现逐年上升趋势,非省项目地区乳腺癌筛查率高于省项目地区,非省项目地区整体呈逐年上升趋势,省项目地区近3年保持相对稳定(P<0.001)。非省项目地区活检遵从率为29.56%,低于省项目地区的61.43%(P<0.001)。非省项目地区乳腺彩色超声可疑检出率和阳性检出率、乳腺X线检查可疑检出率和阳性检出率、活检阳性检出率均高于省项目地区(P均<0.05)。非省项目地区乳腺癌检出率为104.04/10万,低于省项目地区的171.83/10万(P<0.001)。3年间年广东省乳腺癌筛查早诊率呈逐年上升的趋势,非省项目地区早诊率为51.65%,低于省项目地区早诊率的64.22%(P<0.001)。3年间乳腺癌治疗率均超过90%,非省项目地区治疗率为96.20%,低于省项目地区的98.35%(P<0.001)。[结论]广东省 “两癌” 筛查省项目地区乳腺癌筛查组织病理学检查遵从率、早诊率及治疗率等核心质量指标更优,筛查-治疗衔接更为紧密,提示规范化管理与财政支持是提升乳腺癌筛查质量的关键。 |

| Abstract: [Purpose] To analyze the current status of breast cancer screening in provincial unified management areas (hereinafter referred to as “provincial project areas”) and non-provincial unified management areas (hereinafter referred to as “non-provincial project areas”) of the “Two Cancers” (breast cancer and cervical cancer) screening program in Guangdong Province, and reveal the impact of different screening management models on breast cancer screening outcomes. [Methods] Based on the annual reports of breast cancer screening among women in 21 prefecture-level cities of Guangdong Province from 2021 to 2023, regions were divided into provincial project areas and non-provincial project areas according to the screening management model. The χ2 test was used to compare core indicators such as population screening rate, compliance rate of biopsy, detection rate and early diagnosis rate of breast cancer among women aged 35-64 years old between the two regions. [Results] From 2021 to 2023, the population screening rate of breast cancer among women of eligible age in Guangdong Province showed a year-by-year increasing trend. The screening rate in non-provincial project areas was higher than that in provincial project areas, with non-provincial project areas showing an overall year-by-year increasing trend, while the rate in provincial project areas remained relatively stable in the past three years (P<0.001). The compliance rate of biopsy in non-provincial project areas was 29.56%, which was lower than 61.43% in provincial project areas (P<0.001). The suspicious detection rate and positive detection rate of breast color Doppler ultrasound, suspicious detection rate and positive detection rate of mammography, and positive detection rate of biopsy in non-provincial project areas were all higher than those in provincial project areas (all P<0.05). The detection rate of breast cancer in non-provincial project areas was 104.04/105, which was lower than 171.83/105 in provincial project areas (P<0.001). The early diagnosis rate of breast cancer screening in Guangdong Province increased year by year during the three years, with 51.65% in non-provincial project areas, lower than 64.22% in provincial project areas (P<0.001). The treatment rate of breast cancer exceeded 90% in both regions during the three years, with 96.20% in non-provincial project areas, lower than 98.35% in provincial project areas (P<0.001). [Conclusion] The provincial project areas of the “Two Cancers” screening in Guangdong Province had better core quality indicators such as compliance rate of histopathological examination, early diagnosis rate and treatment rate of breast cancer screening, as well as closer screening-treatment linkage. This indicates that standardized management and financial support are the keys to improving the quality of breast cancer screening. |
