| 摘 要:[目的] 分析2012—2024年湖南省城市癌症早诊早治项目人群风险评估及临床筛查结果。[方法] 选取40~74岁湖南省常住居民进行癌症风险评估,对评估出的高风险人群开展肺癌、乳腺癌、结直肠癌、上消化道癌和肝癌的临床筛查,计算各癌种的高危率、筛查依从率和阳性病变检出率。[结果] 2012—2024年共完成风险评估475 153人,评估为高风险者208 317人,总体高危率为 43.84%;五大类癌症合计评估出高危癌种389 177人次,完成相应临床筛查 158 982人次,总体筛查依从率为40.85%;男性筛查依从率高于女性(44.03% vs 39.20%);各癌种中女性乳腺癌依从率最高(51.64%),结直肠癌依从率最低(25.25%)。临床筛查共检出阳性病变 11 738 人次,总检出率为 7.38%;其中肺癌检出率最高(14.86%),其后依次为结直肠癌(5.17%)、女性乳腺癌(3.88%)、胃癌(3.10%)、肝癌(1.42%)和食管癌(0.57%)。[结论] 湖南省城市癌症早诊早治项目在高危人群识别和阳性病变发现方面取得了一定成效,但总体筛查依从率仍有提升空间,尤其需重点提高结直肠癌筛查依从性,并加强高龄等重点人群的精准动员与随访管理。 |

| Abstract: [Purpose] To analyze the population risk assessment and clinical screening results of Cancer Screening Program in Urban Areas of Hunan Province from 2012 to 2024. [Methods] Permanent residents aged 40 to 74 years old in Hunan Province were selected for cancer risk assessment, and clinical screenings for lung cancer, breast cancer, colorectal cancer, upper gastrointestinal cancer and liver cancer were conducted for the identified high-risk population. The high-risk rate, screening compliance rate and detection rate of positive lesions for each cancer type were calculated. [Results] From 2012 to 2024, a total of 475 153 participants completed risk assessment, among whom 208 317 were assessed as high-risk, with an overall high-risk rate of 43.84%. A total of 389 177 person-times of high-risk cancer types were identified across the five major categories of cancer, and 158 982 person-times of corresponding clinical screening were completed, with an overall screening compliance rate of 40.85%. The screening compliance rate in males was higher than that in females(44.03% vs 39.20%). Among all cancer types, female breast cancer had the highest compliance rate (51.64%), while colorectal cancer had the lowest (25.25%). A total of 11 738 person-times of positive lesions were detected by clinical screening, with an overall detection rate of 7.38%. Among them, lung cancer had the highest detection rate (14.86%), followed by colorectal cancer (5.17%), female breast cancer (3.88%), gastric cancer (3.10%), liver cancer (1.42%) and esophageal cancer (0.57%). [Conclusion] The Urban Cancer Early Diagnosis and Treatment Program in Hunan Province has achieved certain effectiveness in identifying high-risk populations and detecting positive lesions. However, the overall screening compliance rate still needs to be improved, particularly by enhancing compliance with colorectal cancer screening and strengthening targeted mobilization and follow-up management among key populations such as older adults. |
