| 摘 要:[目的] 分析 2014—2023 年江苏省扬中市肺癌发病和死亡的流行特征。[方法]根据中国肿瘤登记平台中扬中市肿瘤登记地区2014—2023年的肺癌发病和死亡数据及公安人口数据,按性别、年龄分层计算各年份粗发病(死亡)率、中国人口标化发病(死亡)率[简称中标发病(死亡)率];采用Joinpoint回归模型分析发病率与死亡率的平均年度变化百分比(average annual percentage change,AAPC),分析变化趋势,检验水准α=0.05。[结果] 2014—2023年扬中市新发肺癌病例数共1 767例,死亡1 645例。全人群肺癌粗发病率为62.64/10万(男性82.94/10万、女性43.21/10万),中标发病率为27.62/10万;全人群肺癌粗死亡率为58.31/10万(男性88.16/10万、女性29.76/10万),中标死亡率为23.31/10万。年龄别分布显示,肺癌发病率在0~49岁处于低水平,50岁后随年龄增长快速上升且在80~84岁组达峰值;死亡率在0~54岁处于低水平,55岁后随年龄增长快速上升,全人群及男性在80~84岁组达峰值,女性在85岁及以上组达峰值。2014—2023年肺癌中标发病率显著上升,从17.53/10万上升至36.90/10万(AAPC=6.86%,P<0.01),其中女性中标发病率呈现显著上升趋势(AAPC=13.69%,P<0.01),男性中标发病率相对平稳(AAPC=3.15%,P=0.10)。肺癌中标死亡率从13.33/10万上升至20.15/10万,无明显波动(AAPC=0.60%,P=0.82)。男女性中标死亡率均无明显变化趋势(P均>0.05)。[结论]2014—2023年扬中市肺癌发病呈显著上升趋势,男性疾病负担高于女性且随年龄增长加重;需结合本地流行特征,针对性开展控烟、环境治理、早筛早治等综合防控措施,降低肺癌发病与死亡风险。 |

| Abstract:[Purpose] To analyze the epidemiological characteristics of lung cancer incidence and mortality in Yangzhong City, Jiangsu Province from 2014 to 2023. [Methods] Based on the lung cancer incidence and mortality data of Yangzhong City from 2014 to 2023 retrieved from the National Cancer Registration Platform and the household registration data from the public security department, the crude incidence (mortality) rate and age-standardized incidence (mortality) rate by Chinese standard population (ASIRC/ASMRC) were calculated by sex and age stratification. Joinpoint regression model was used to analyze the average annual percentage change (AAPC) of incidence and mortality, with a significance level of α=0.05. [Results] A total of 1 767 new lung cancer cases and 1 645 deaths were reported in Yangzhong City from 2014 to 2023. The crude incidence rate of lung cancer in the total population was 62.64/105 (82.94/105 in male and 43.21/105 in female), and ASIRC was 27.62/105. The crude mortality rate was 58.31/105 (88.16/105 in male and 29.76/105 in female), and ASMRC was 23.31/105. Age-specific distribution showed that the incidence rate was at a low level in the age group of 0~49 years old , increased rapidly after 50 years old, and peaked in the age group of 80~84 years old. The mortality rate was at a low level in the age group of 0~54 years old, increased rapidly after 55 years old. The peak mortality rate was in the age group of 80~84 years old for the total population and male, and in the age group of ≥85 years old for female. From 2014 to 2023, ASIRC of lung cancer in the total population increased significantly from 17.53/105 to 36.90/105 (AAPC=6.86%, P<0.01), among which ASIRC in female increased significantly (AAPC=13.69%, P<0.01), while that in male remained relatively stable (AAPC=3.15%,P=0.10). The ASMRC of lung cancer in the total population increased from 13.33/105 to 20.15/105, with no significant change (AAPC=0.60%, P=0.82), and there was no obvious trend in both male and female (both P>0.05). [Conclusion] From 2014 to 2023, the incidence of lung cancer in Yangzhong City showed a significant upward trend. The disease burden was heavier in male than that in female and increased with age. Comprehensive prevention and control measures such as tobacco control, environmental governance, and early screening and treatment should be targeted based on local epidemiological characteristics to reduce the incidence and mortality risks of lung cancer. |
