| 摘 要：［目的］分析2008—2019年辽宁省庄河市胃癌筛查人群中幽门螺杆菌（Helicobacter pylori，Hp）感染状况及其变化趋势。［方法］ 纳入2008年1月至2019年12月期间在庄河市进行胃癌筛查的受检者，采用ELISA法检测血清抗Hp抗体-IgG，判定Hp感染情况。所有受检者均接受胃镜检查，常规HE染色及病理组织学诊断。采用Joinpoint软件进行趋势检验，计算年均变化百分比（average annual percentage change，AAPC）和95%置信区间（confidence interval，CI）。［结果］ 2008—2019年，庄河市接受筛查且进行Hp抗体-IgG检测的受检者共计13 552人，其中男性5 794人，女性7 758人，受检者年龄为40~69岁，中位年龄为55岁。Hp感染阳性率为36.33%。男性受检者Hp感染阳性者2 183例（37.68%），女性受检者Hp感染阳性者2 741例（35.33%），男性高于女性（P=0.005）。40~49岁和50~59岁年龄组间Hp感染阳性率高于60~69岁年龄组。肠化型萎缩性胃炎和胃癌患者Hp感染阳性率分别为48.51%和56.72%，高于基本正常人群（32.33%）。2008—2019年间受检人群Hp感染阳性率呈明显的降低趋势（AAPC=-6.47%，95%CI：-10.54%~-2.20%，P<0.05）。［结论］庄河市筛查人群Hp感染与年龄、性别、胃疾病状态具有一定相关性，12年间该地区Hp感染阳性率呈降低趋势。 |

| Abstract： ［Purpose］ To analyze the trend of Helicobacter pylori (Hp) infection rate among screening population in Zhuanghe City of Liaoning Province. ［Methods］ Individuals underwent gastric cancer screening in Zhuanghe City of Liaoning Province from January 2008 to December 2019 were included. During the screening serum anti-Hp antibody-IgG was detected by ELISA， gastroscopy was performed for all subjects， biopsy/histopathological diagnosis was made for positive cases. The trend was analyzed using the Joinpoint regression model， and the average annual percentage change and confidence interval were calculated. ［Results］ A total of 13 552 individuals(5 794 males and 7 758 females) with a mean age of 55 years old (40 to 69 years old) were included. Among 13 552 screenees Hp infection was positive in 2 183 male subjects (37.68%) and 2 741 female subjects (35.33%) (P=0.005). The positive rates of Hp infection in the age groups of 40~49 and 50~59 years old were higher than that in the age group of 60~69 years old. The positive rates of Hp infection in patients with atrophic gastritis and gastric cancer were higher than that in general healthy subjects (48.51% and 56.72% vs 32.33%). The positive rates of Hp infection in the periods 2008—2019 showed a decreasing trend(AAPC=-6.47%， 95%CI： -10.54% ~ -2.20%， P<0.05）. ［Conclusion］ Hp infection rate in the screening population in Zhuanghe City is associated with age， gender and gastric disease status， showing a decreasing trend from 2008 to 2019. |
