      摘 要:[目的] 分析2009—2020年海宁市肺癌的发病、死亡情况以及变化趋势。[方法] 收集海宁市肿瘤登记地区2009—2020年肺癌的发病、死亡信息,并用Excel、SPSS和Joinpoint软件分析计算肺癌发病(死亡)例数、粗发病率(死亡率)、中标率、世标率、0~74岁累积发病(死亡)率、35~64岁截缩发病(死亡)率、年龄别发病率(死亡率)以及年度变化百分比(APC)等指标。[结果] 2009—2020年海宁市肺癌新发病例共计7 120例,男女比为1.64∶1;因肺癌死亡病例共计4 372例,男女比为2.48∶1。2009—2020年海宁市肺癌粗发病率为87.66/10万,中标发病率为43.24/10万,世标发病率为42.38/10万,0~74岁累积率为5.19%,35~64岁截缩率为61.95/10万。2009—2020年海宁市肺癌粗死亡率为53.83/10万,中标死亡率为24.12/10万,世标死亡率为23.70/10万,0~74岁累积率为2.74%,35~64岁截缩率为23.69/10万。2009—2020年海宁市肺癌中标发病率呈上升趋势(APC=7.18%,95%CI:4.85%~9.57%,P<0.05);肺癌中标死亡率呈下降趋势(APC=-2.02%,95%CI:-2.96%~-1.08%,P<0.05)。2009—2020年海宁市肺癌年龄别发病率随着年龄增长而增加,0~39岁处于较低水平,在75~79岁组到达高峰,为419.71/10万。年龄别死亡率增长情况与发病率类似,在0~44岁期间处于较低水平,随后增长急剧,在80~84岁组达到高峰,为387.00/10万。除少数年龄组以外,男性年龄别发病率和死亡率均高于女性。[结论] 2009—2020年海宁市肺癌发病率逐年上升,男性发病率高于女性,女性发病上升趋势明显。应加强对男性和中老年等高危人群的健康宣教和肺癌防治工作,提高早诊率和治愈率。

      Abstract:[Purpose] To analyze the incidence, mortality and changing trends of lung cancer in Haining City from 2009 to 2020. [Methods] The incidence and death information of lung cancer in the cancer registration areas of Haining City from 2009 to 2020 were collected, and the incidence (death) cases, crude incidence (mortality) rate, age-standardized incidence and mortality by Chinese standard population (ASIRC and ASMRC) and by world standard population (ASIRW and ASMRW),cumulative incidence (mortality) rate of 0~74 years old, truncated incidence (mortality) rate of 35~64 years old, age-specific incidence and mortality rate, and annual percentage change (APC) of incidence and mortality rate were analyzed and calculated by Excel, SPSS and Joinpoint software. [Results] From 2009 to 2020, there were a total of 7 120 new cases of lung cancer in Haining City, with a male to female ratio of 1.64∶1. There were a total of 4 372 deaths from lung cancer, with a male to female ratio of 2.48∶1. From 2009 to 2020, the crude incidence rate of lung cancer in Haining City was 87.66/105, the ASIRC and ASIRW were 43.24/105 and 42.38/105,respectively; the cumulative rate of 0~74 years old was 5.19%, and the truncated rate of 35~64 years old was 61.95/105. The crude mortality rate of lung cancer in Haining City from 2009 to 2020 was 53.83/105, the ASMRC and ASMRW were 24.12/105 and 23.70/105 respectively; the cumulative rate of 0~74 years old was 2.74%, and the truncated rate of 35~64 years old was 23.69/105. From 2009 to 2020, the ASIRC rate of lung cancer in Haining City showed an upward trend (APC=7.18%, 95%CI: 4.85%~9.57%, P<0.05). The ASMRC of lung cancer in Haining City showed a decreasing trend from 2009 to 2020 (APC=-2.02%, 95%CI: -2.96%~-1.08%, P<0.05). From 2009 to 2020, the age-specific incidence rate of lung cancer in Haining increased with age. The incidence of lung cancer was at a low level in age groups of 0~39 years old, and reached a peak of 419.71/105 in the age group of 75~79 years old. The growth of age-specific mortality was similar to that of incidence rate. It was at a low level during the period from 0 to 44 years old, and then increased sharply, reaching a peak of 387.00/105 in the age group of 80~84 years old. Except for a few age groups, the age-specific incidence and mortality rates of male were higher than that of female.[Conclusion] The incidence rate of lung cancer increased year by year from 2009 to 2020 in Haining. The incidence rate of lung cancer in male was higher than that in female, and the incidence of lung cancer in female increased significantly. We should strengthen health education and lung cancer prevention and treatment for high-risk groups such as men and middle-aged and elderly people, in order to improve early diagnosis and cure rates.
