      摘 要:[目的] 描述分析2007―2017年天津市大港地区甲状腺癌发病和死亡情况。[方法] 提取2007―2017年天津市肿瘤登记系统和死因登记系统中户籍为大港地区的肿瘤病例资料进行分析,计算甲状腺癌发病率、死亡率及年度变化百分比(annual percentage change,APC),并根据2000年中国标准人口构成计算标化率。[结果] 2007―2017年,天津市大港地区全人群新发甲状腺癌1535例,粗发病率34.59/10万;其中男性386例,粗发病率16.88/10万;女性1149例,粗发病率53.42/10万。全人群甲状腺癌死亡27例,粗死亡率0.61/10万;其中男性14例,粗死亡率0.61/10万;女性13例,粗死亡率0.60/10万。2007―2014年,全人群甲状腺癌标化发病率APC为33.0%(P<0.05),男性标化发病率APC为41.9%(P<0.05),女性标化发病率APC为29.1%(P<0.05)。2014—2017年,全人群甲状腺癌标化发病率APC为-7.1%(P>0.05),男性标化发病率APC为-10.5%(P>0.05),女性标化发病率APC为-5.8%(P>0.05)。2007―2017年全人群甲状腺癌标化死亡率APC值为3.5%(P>0.05)。[结论] 2007―2014年天津市大港地区甲状腺癌发病率迅速增长,2014―2017年甲状腺癌发病率出现了下降的新趋势,但仍处于较高水平。2007―2017年甲状腺癌死亡率呈现出缓慢上升的趋势。应进一步探索甲状腺癌高发的原因,采取更有针对性的预防措施。加强甲状腺癌健康教育工作,提高居民甲状腺癌相关知识知晓率。

      Abstract:[Purpose] To analyze the incidence and mortality of thyroid cancer in Dagang District of Tianjin from 2007 to 2017. [Methods] Cases diagnosed as thyroid cancer in 2007—2017 in Tianjin Dagang District were extracted from the population-based database of Tianjin cancer registry system and death registry system. Incidence,mortality and annual percentage change(APC) of thyroid cancer were calculated. Standard population of China in 2000 was used for age-standardized incidence and mortality rates. [Results] From 2007 to 2017,there were 1535 new cases of thyroid cancer in Dagang district of Tianjin,with the crude incidence rate of 34.59/105. Among them,386 cases were male,with the crude incidence of 16.88/105,and 1149 cases were female,with the crude incidence of 53.42/105. There were 27 deaths of thyroid cancer in the whole population,with the crude mortality of 0.61/105. Among them,14 cases were male,with the crude mortality of 0.61/105,and 13 cases were female,with the crude mortality of 0.60/105. From 2007 to 2014,the APC of age-standardized incidence of thyroid cancer was 33.0%(P<0.05) in the whole population,41.9%(P<0.05) in male and 29.1%(P<0.05) in female. From 2014 to 2017,the APC of age-standardized incidence of thyroid cancer was -7.1%(P>0.05) in the whole population,-10.5%(P>0.05) in male and -5.8%(P>0.05) in female. From 2007 to 2017,the APC of age-standardized mortality rate of thyroid cancer in the whole population was 3.5%(P>0.05).[Conclusion] The incidence of thyroid cancer in Dagang District of Tianjin increased rapidly from 2007 to 2014,and showed a new trend of decline from 2014 to 2017,but it was still at a high level. The mortality rate of thyroid cancer showed a slow upward trend from 2007 to 2017. We should further explore the causes of high incidence of thyroid cancer and take more targeted preventive measures. The health education of thyroid cancer should be strengthened to improve the awareness rate of thyroid cancer among residents.
