      摘 要:[目的] 探讨乳腺癌患者治疗后妊娠对无病生存期及总生存期的影响。 [方法] 检索中国知网、万方、维普、PubMed、Embase、Cochrane Library等数据库,收集关于乳腺癌患者治疗后妊娠对其预后影响的文献,筛选纳入合格文献,并提取风险比值(hazard ratio,HR)及 95% 可信区间(confidence interval,CI)。用RevMan 5.3采用一般倒方差法进行数据分析,得出分析结果的森林图。异质性检验通过Q统计量和I2值进行,根据结果选用相应的效应模型绘制漏斗图以评价发表偏倚。[结果] 共有8篇文献符合纳入标准,其中6项研究纳入无病生存期分析,5项研究纳入总生存期分析。分析结果显示乳腺癌治疗后妊娠组患者的复发风险与非妊娠组差异无统计学意义(HR=0.72,95%CI:0.48~1.06,P=0.10);而死亡风险较未妊娠组明显降低(HR=0.51,95%CI:0.32~0.82,P=0.006)。[结论] 乳腺癌患者治疗后妊娠可能对无病生存期无不良影响,而对总生存期可能是一种保护性因素。

      Abstract:[Purpose] To investigate the effect of pregnancy on disease-free survival and overall survival in patients with breast cancer after treatment. [Methods] The databases of Chinese Knowledge Network,Wanfang,Weipu,PubMed,Embase,Cochrane Library were searched for studies about the effect of pregnancy on the prognosis of breast cancer patients after treatment. Eligible studies were screened and included,hazard ratio(HR) and 95% confidence interval(CI) were extracted. RevMan 5.3 was used for data analysis using the general inverse variance method to obtain the forest map. Heterogeneity tests were conducted using Q statistics and I2 values,and corresponding effect models were used to draw funnel plots to evaluate publication bias. [Results] Eight articles met the inclusion criterias,of which 6 studies were included in disease-free survival analysis and 5 studies were included in overall survival analysis. The analysis showed that the risk of recurrence in the pregnant women after breast cancer treatment was not statistically different from that of the non-pregnant group(HR=0.72,95%CI:0.48~1.06,P=0.10);the risk of death was significantly lower than that of the non-pregnant group(HR=0.51,95%CI:0.32~0.82,P=0.006). [Conclusion] Pregnancy after breast cancer treatment may have no adverse effect on disease-free survival and may be a protective factor for overall survival.
