| 摘 要：［目的］ 探讨危险度评估问卷调查联合粪便隐血试验为方法的大肠癌筛查对于大肠癌患者生存时间的影响，为大肠癌筛查的实施及策略的制定提供依据。［方法］ 根据2013年上海市嘉定区肿瘤登记系统的患者信息匹配大肠癌筛查系统数据，采用Kaplan-Meier法计算生存率及绘制生存曲线，Log-rank检验及多因素Cox回归模型被用于分析影响患者生存的因素。［结果］ 单因素分析结果提示，TNM临床分期、肿瘤位置、大肠癌筛查与大肠癌患者生存有显著关联（P<0.05），其中TNM临床分期早期、肿瘤位于直肠及参加大肠癌筛查是提高患者生存率的有利因素。多因素Cox回归模型结果提示TNM临床分期及大肠癌筛查是影响患者生存的独立因素。分层分析结果表明，参加大肠癌筛查对于个体特征及肿瘤临床特征不同的患者增加生存率的意义是不同。［结论］ 参加危险度评估问卷调查联合粪便隐血试验为方法的大肠癌筛查是提升大肠癌患者生存率的独立且有利因素。 |

| Abstract：［Purpose］ To assess the prognosis of colorectal cancer patients who were detected by screening. ［Methods］ The data of colorectal cancer patients were collected from Jiading District Cancer Registration System and colorectal cancer screening system of 2013. Kaplan-Meier curve was used to determine the survival rate；Log-rank test and the multivariate Cox regression model were used to analyze the influencing factors of patient survival. ［Results］ Univariate analysis showed that early TNM clinical stage，tumor located in the rectum，and participating in colorectal cancer screening were favorable factors for prolonging the survival of patients(P<0.05). Multivariate Cox regression model results suggest that TNM clinical stage and colorectal cancer screening were independent factors affecting patient survival. The results of stratified analysis showed that participation in colorectal cancer screening was different for patients with different characteristics and clinical features of tumors. ［Conclusion］ Participation in colorectal cancer screening is an independent and beneficial factor for prolonging the survival of patients with colorectal cancer. |
