      摘 要:[目的] 分析江苏省昆山市1981~2015年去肝癌死因可增加期望寿命和肝癌所致的潜在减寿年时间趋势。[方法] 1981~2015年肝癌死亡病例来源于死因监测,计算去肝癌死因可增加期望寿命和潜在减寿年;去肝癌死因可增加期望寿命=去肝癌死因期望寿命-平均期望寿命。用中国2000年第五次人口普查的年龄结构计算分性别和年份的年龄标化潜在减寿年。使用年度变化百分比(aAPC)评价去肝癌可增加期望寿命和潜在减寿年在年份之间的变化趋势。[结果] 总人群中去肝癌死因可增加期望寿命由1981年的0.55岁下降到2015年的0.46岁,下降趋势有统计学意义(aAPC=-0.6%,95%CI:-1.0%~-0.2%);女性人群中(aAPC=-0.7%,95%CI: -1.2%~-0.2%)去肝癌死因可增加期望寿命呈现下降趋势,而男性人群(aAPC=-0.4%,95%CI:-0.9%~0.1%)则无明显趋势变化。肝癌所致的标化潜在减寿年在总人群(aAPC=-3.2%,95%CI:-3.7%~-2.7%)、男性人群(aAPC=-2.8%,95%CI:-3.3%~-2.3%)和女性人群(aAPC=-4.5%,95%CI:

      Abstract:[Purpose] To examine impact and temporal trends of liver cancer death on life expectancy and potential years of life lost(PYLL)in Kunshan City,Jiangsu Province,1981 to 2015. [Methods] Data were collected from vital registry of Kunshan City. Liver cancer-free life expectancy and the PYLL were calculated. Increased life expectancy by liver cancer-free was defined as liver cancer-free life expectancy minus average life expectancy. The fifth Chinese population census in 2000 was used to calculate age-standardized PYLL. Average annual percentage changes(aAPC) and 95% confidence interval(CI)were used to examine the temporal trend of increased liver cancer-free life expectancy and PYLL. [Results] During 1981 to 2015,the increased liver cancer-free life expectancy was substantially decreased(aAPC=-0.6%,95%CI:-1.0%~-0.2%),and there was significant decreasing trend observed for females(aAPC=-0.7%,95%CI:-1.2%~-0.2%),but not for males(aAPC=-0.4%,95%CI:-0.9%~0.1%). Moreover,there was substantial decline for the age-standardized PYLL among both sexes(aAPC=-3.2%,95%CI:-3.7%~-2.7%),among males(aAPC=-2.8%,95%CI:-3.3%~-2.3%)and females(aAPC=-4.5%,95%CI:-5.4%~-3.6%).
