| 摘 要：［目的］ 探讨PARP-1的表达与转移性乳腺癌患者预后的关系及PARP-1对含铂类方案化疗敏感性的影响。［方法］ 收集转移性乳腺癌患者的临床病理资料和肿瘤组织蜡块，免疫组化方法检测其术前PARP-1的表达水平。分析原发灶PARP-1表达水平与转移性乳腺癌患者预后及含铂方案化疗疗效的关系。［结果］转移性乳腺癌患者原发灶PARP-1表达阳性率为73.20%，PARP-1高表达患者较低表达患者内脏转移部位更广泛（P=0.025）、CXCR4表达水平高（P=0.003）。PARP-1高表达者较低表达者含铂化疗敏感性差（P＝0.000）。PARP-1高表达的患者无疾病生存期（DFS）和总生存期（OS）较低表达者短，但差异均无统计学意义。［结论］ 转移性乳腺癌患者原发灶PARP-1蛋白高表达者较低表达者含铂化疗疗效差。 |

| Abstract：［Purpose］ To investigate the expression of PARP-1 in primary tumor of metastatic breast cancer and its relation to therapeutic effects and prognosis of patients.［Methods］ The clinicopathological data of 92 patients with metastatic breast cancer，who received palliative chemotherapy of platinum-containing regimens，were enrolled in the study. The expression of PARP-1 was detected by immunohistochemistry in the primary tumors. The relationship of the expression level of PARP-1 with the therapeutic effects and prognosis of patients were analyzed. ［Results］ The positive rate of PARP-1 expression was 73.20% in patients with metastatic breast cancer. Patients with higher expression of PARP-1 also had higher expression of CXCR4(P=0.003)，and were prone to have more internal organ metastasis than patients with lower expression of PARP-1(P=0.025). Patients with high PARP-1 expression were less sensitive to platinum based chemotherapy than those with low PARP expression(P=0.000). Disease free survival (DFS) and overall survival (OS) showed a decreasing trend in patients with high expression of PARP-1(P=0.076 and P=0.694). ［Conclusion］ Higher expression of PARP-1 protein is associated with poorer therapeutic effect to platinum based chemotherapy in patients with metastatic breast cancer. |
