| 摘 要：［目的］ 估计山东省恶性肿瘤的发病与死亡水平。［方法］ 2015年共收到21个肿瘤登记处上报的2012年肿瘤登记数据，通过审核和评估，共15 个肿瘤登记处的数据符合入选标准。将入选的登记处按地区（城乡）、性别以及年龄别、肿瘤别发病率和死亡率分层，结合2012年山东省人口数据，估计山东省恶性肿瘤合计和主要肿瘤的发病、死亡情况。标化率分别采用2000年全国人口普查人口和Segi’s标准人口结构为标准。［结果］ 山东省15个肿瘤登记处共覆盖登记人口17 189 988人（其中城市7 486 039人，农村9 703 949人）。恶性肿瘤病理诊断比例为66.12%，只有死亡证明书比例为2.93%，死亡发病比为0.60。估计全省2012 年新发恶性肿瘤病例253 060例，死亡病例157 750例。山东省恶性肿瘤发病率为263.86/10万（男性303.29/10 万、女性223.23/10 万）。中国人口标化率(中标率)为192.42/10万，世界人口标化率(世标率)为189.50/10 万，累积率（0～74岁）为22.07%。城市地区发病率为267.64/10 万，中标发病率为195.27/10万，农村地区发病率为262.32/10万，中标发病率为191.26/10万。山东省恶性肿瘤死亡率为164.47/10万（男性207.42/10万，女性120.23/10万），中标死亡率为117.54/10万，世标死亡率为116.90/10万，累积死亡率（0～74岁）为13.53%。城市地区死亡率为141.59/10万，中标死亡率为101.17/10万，农村地区恶性肿瘤死亡率为173.79/10万，中标死亡率为124.20/10万。肺癌、胃癌、肝癌、食管癌、结直肠癌、女性乳腺癌、脑瘤、白血病、膀胱癌和胰腺癌是山东省主要的恶性肿瘤，约占全部新发病例的82.12%。肺癌、胃癌、肝癌、食管癌、结直肠癌、乳腺癌、胰腺癌、脑瘤、白血病和淋巴瘤是主要的肿瘤死因，约占全部肿瘤死亡病例的89%。［结论］ 目前山东省肿瘤疾病负担严重，应继续加强肿瘤防治工作。 |

| Abstract：［Purpose］ To estimate cancer incidence and mortality in Shandong province. ［Methods］ Twenty one cancer registries in Shandong province submitted new cancer cases and deaths in 2012. All the data were checked and evaluated based on the National Central Cancer Registry(NCCR) criteria of data quality， and qualified data from 15 registries were used for cancer statistics analysis as provincial estimation. The pooled data were stratified by area(urban/rural)，gender，age group and cancer type. New cancer cases and deaths were estimated using age-specific rates and corresponding provincial population in 2012.The Chinese census in 2000 and Segi’s population were applied for age-standardized rates. ［Results］ The qualified 15 cancer registries covered populations of 17 189 988(7486039 in urban and 9703949 in rural areas). The percentage of cases morphologically verified(MV%) and death certificate-only cases(DCO%) were 66.12% and 2.93%，respectively，and the mortality to incidence rate ratio(M/I) was 0.60. A total of 253 060 new cancer cases and 157 750 cancer deaths were estimated in Shandong province in 2012. The incidence was 263.86/105(303.29/105 in male，223.23/105 in female)，age-standardized incidence by Chinese standard population(ASR China) and by world standard population(ASR world) were 192.42/105 and 189.50/105 with the cumulative incidence rate(0~74 age years old) of 22.07%. The cancer incidence，ASR China in urban areas were 267.64/105，195.27/105，while those in rural areas were 262.32/105，191.26/105 respectively. The cancer mortality was 164.47/105(207.42/105 in male，120.23/105 in female)，age-standardized mortality by Chinese standard population and by world standard population were 117.54/105 and 116.90/105，and the cumulative mortality rate(0~74 age years old) was 13.53%. The cancer mortality，ASR China and ASR world were 141.59/105，101.17/105 and 100.33/105 in urban areas，while those in rural areas were 173.79/105，124.20/105 and 123.64/105，respectively. Cancer of lung，stomach，liver，esophagus，colorectum，female breast，brain tumor，leukemia，bladder and pancreas were the most common cancers，accounting for about 82.12% of all cancer new cases. Lung cancer，stomach cancer，liver cancer，esophageal cancer，colorectal cancer，emale breast cancer，pancreatic cancer，brain tumor，leukemia and lymphoma were the leading causes of cancer death，accounting for about 89.01% of all cancer deaths. The cancer spectrum showed difference between urban and rural，male and female in incidence and mortality rates. ［Conclusions］ The disease burden of cancer is serious in Shandong province，so that，cancer prevention and control should be enhanced including of health education，health promotion，cancer screening and cancer care services in Shandong province. |
