| 摘 要：［目的］ 了解长沙市城市妇女乳腺癌发病情况，探讨乳腺癌筛查模式价值。［方法］2012~2015年对长沙市40~69岁城市女性进行乳腺癌筛查，采用乳腺癌高危评估系统初筛出高危人群，40~44岁高危人群采用彩色超声检查，45岁及以上采用彩色超声检查结合X线摄影检查，以BI-RADS为Ⅳ级和Ⅴ级作为阳性结果。［结果］ 共有69 532人参加了乳腺癌危险因素问卷调查，13 805例(19.9%)被评为高危人群。1906人接受了单独彩色超声检查，4133人接受了彩色超声结合X线摄影检查。超声阳性检出率为774/10万；X线摄影阳性检出率为560/10万。两种筛查方法一致性检验Kappa系数为0.394（P<0.05）。［结论］ 采用高危风险评估系统进行初筛，按照不同年龄特点选择不同的筛查方法的乳腺癌早诊早治模式为我国的乳腺癌防控工作提供了经验。 |

| Abstract：［Purpose］ To investigate prevalence of female breast cancer in Changsha city，and to explore a new mode of breast cancer screening. ［Methods］ The urban women with 40 to 69 years old in Changsha city from 2012 to 2015 were enrolled into breast cancer screening. Risk assessment system was used to find out high-risk persons. The high-risk person with 40 to 44 years old received ultrasound examination，while high-risk person with 45 years old and above received ultrasound combined with X-ray photography. Positive result defined as BI-RADS grade Ⅳ and Ⅴ. ［Results］ A total of 69 532 women participated in questionnaire survey for high- risk of breast cancer，13 805 women were rated as high-risk groups with accounting for 19.9%. A total of 1906 high-risk person received screening by ultrasound alone，and 4133 high-risk person received screening by ultrasound combined with X-ray photography. Positive detection rate of ultrasound was 774/105，and that with X-ray photography was 560/105. Consistency analysis showed that Kappa coefficient of two methods was 0.394. ［Conclusion］ The early diagnosis and treatment mode for breast cancer using high-risk risk assessment system and screening method based on different ages may provide helpful for Chinese breast cancer prevention and control. |
