| This article analyzed the development process of early diagnosis and treatment of cancer in Nantong City, systematically tracing its evolution from the prevention and control of liver cancer in Qidong to the joint screening of multiple cancer types in rural and urban areas. In 1972, Qidong took the lead in establishing a tumor registration and reporting system in rural areas nationwide, adopting α-fetoprotein detection for liver cancer screening, laying the foundation for subsequent high-risk population screening for liver cancer. In 1983, the world''s earliest randomized controlled trial for neonatal hepatitis B vaccination was initiated in Qidong,, providing crucial evidence for the national hepatitis B vaccination strategy. Since 2006, Qidong and Haimen have successively implemented early diagnosis and treatment projects for liver cancer in rural areas, with a total of 137,787 follow-ups and 485 confirmed cases of liver cancer, with an early diagnosis rate of 82.06% and a treatment rate of 92.78%. In 2014, Nantong City launched an urban early diagnosis and treatment project for cancer, covering five major cancer types, with a total of 189,506 risk assessments and 325 confirmed cases. The five-year survival rate among the screened population was highest for breast cancer and colorectal cancer. In terms of organizational management, Nantong City has established a trinity medical and preventive service model integrating "disease control center-hospital-community health institutions", forming an integrated framework that includes CDC quality control coordination, hospital diagnosis support, and grassroots assessment and follow-up. In the future, it is necessary to promote the combination of opportunistic screening and organized screening, utilize artificial intelligence technology to achieve multi-source data interconnection and intelligent risk assessment, and promote precision screening, intelligent services, and equalization of resources. More than half a century of cancer screening practice in Nantong city has formed a multi-project parallel early screening system covering major high-incidence cancer types, providing scientific basis for the construction of a prefecture-level cancer prevention and control system. |
