| [Purpose]: To analyze the screening effectiveness and cost-effectiveness of a liver cancer high-risk cohort in the Huai River Basin of Sheyang County from 2017 to 2022. [Methods]: A high-risk cohort was established from individuals with a prior positive hepatitis B virus surface antigen. If the sample size was insufficient, cluster sampling was used to recruit permanent residents aged 35–70 years to participate in questionnaire surveys and hepatitis B virus surface antigen testing, supplementing the cohort to 2,000 individuals. Annual alpha-fetoprotein (AFP) testing and ultrasound examinations were performed. Incidence density(ID), early diagnosis rate, and survival rate were compared among participants with different characteristics, and health economic evaluation was conducted based on direct screening costs. [Results]: The baseline high-risk cohort included 2,492 individuals, with a total of 12,405 person-times screened. A total of 80 liver cancer cases were detected. The incidence density in males was significantly higher than that in females, and a significant age-related incidence trend was observed. There were 42 early-stage cases, with an early diagnosis rate of 52.5% and a 5-year survival rate of 43.7%. The costs per detected case and per detected early-stage case were 52,150 RMB and 99,333 RMB, respectively, and the early detection cost coefficient was 0.64. [Conclusion]: The liver cancer high-risk cohort screening in the Huai River areas of Sheyang County achieved relatively high early diagnosis and 5-year survival rates for liver cancer, demonstrating favorable health economic value. Future efforts should strengthen screening among the male population and promote early intervention to improve patient survival. |
