      摘要:[目的] 收集2014-2023年南通市城市癌症早诊早治项目肺癌筛查与随访结果并进行危险因素分析。[方法] 根据城市癌症早诊早治项目技术方案,收集整理2014-2023年南通市城市癌症早诊早治项目肺癌筛查人群的风险评估、临床筛查及随访信息,并与南通市恶性肿瘤和生命统计数据系统进行匹配获得筛查人群自参与项目起至2025年12月31日期间肺癌发病与死亡结局。计算高危率、筛查参与率、阳性发病率、死亡率与观察生存率(OSR)并对其在不同人口学特征中的差异进行单因素和多因素分析。单因素分析采用χ^2检验,多因素分析采用logistic回归模型。生存分析中,通过Kaplan-Meier法计算观察生存率,利用Log-rank检验进行单因素分析、Cox回归进行多因素分析。单因素分析有意义(P<0.05)的变量将纳入多因素分析。[结果] 本研究最终纳入184852人。其中,男性74914人,占40.53%;女性109938人,占59.47%。发现肺癌高危对象31655人,高危率为17.12%,其中15690人进行了临床筛查,筛查参与率为49.57%。2014-2023年间,共发生3250(1.76%)例肺癌病例和979(0.53%)例肺癌死亡病例,肺癌患者的1年、3年和5年OSR分别为80.26%、73.75%和69.62%。风险评估模型的灵敏度为31.51%,特异度为83.13%。分析结果显示,男性、年老、吸烟和有呼吸系统疾病史人群的肺癌阳性发病率和死亡率较高、OSR较低。[结论] 南通市肺癌筛查人群的总体高危率较低,阳性发病和死亡率较高,肺癌患者的生存率较高。男性、年老和吸烟史和有呼吸系统疾病史人群为肺癌防治的重点人群。应积极开展宣教,提升居民健康素养,培养肺癌早诊早治意识,减少因肺癌治疗所带来的健康和社会负担。

      Abstract: [Purpose] To analyze the lung cancer screening and follow-up results of the Urban Early Detection and Treatment Program for Cancer in Nantong City from 2014 to 2023, a risk factor analysis was performed as well. [Method] According to the technical protocol of the Urban Cancer Early Detection and Treatment Program, the baseline risk assessment, clinical screening, and follow-up information from 2014 to 2023 were collected. Status and survival outcome of lung cancer were obtained via cancer registry and vital statistics system, with follow-up from the date of enrollment to December 31, 2025. The high-risk rate, the screening rate, the cumulative incidence, the mortality rate and the observed survival rate (OSR) were calculated. Univariate analysis was conducted using the chi-square test or the rank-sum test, and multivariate analysis employed logistic regression. In the survival analysis, OSR was calculated using the Kaplan–Meier method. The Log-rank test was used for univariate analysis, and Cox regression was applied for multivariate analysis. Variables that were statistically significant in the univariate analysis (P < 0.05) were included in the multivariate analysis. [Results] Totally 184852 participants were included among whom, 74,914 were male (accounting for 40.53%) and 109938 were female(accounting for 59.47%). There were 31655 cases identified as high risk for lung cancer, accounting for17.12%, among whom 15690 cases received Low-dose CT screening with a compliance rate of 49.57%. From 2014 to 2023, a total of 3250(1.76%) lung cancer cumulative incident cases and 979(0.53%) cases dying of lung cancer were recorded. The 1-year, 3-year and 5-year OSR for lung cancer were 80.26%、73.75% and 69.62%, respectively. The sensitivity of the risk assessment model for lung cancer was 31.51% and the specificity was 83.13%. Male, aging, smoking and history of respiratory diseases were associated with higher lung cancer incidence and mortality, as well as lower OSR. [Conclusion] A paradoxically low high-risk prevalence coupled with high lung cancer detection and mortality rates in Nantong City, alongside improved survival rates for diagnosed patients were observed. Male, aging, smoking and history of respiratory diseases are the key target groups for lung cancer prevention and treatment. Efforts should be actively made to carry out public awareness campaigns, enhance residents'' health literacy, foster awareness of early detection and early treatment of lung cancer, and reduce the health and social burden associated with lung cancer treatment.
