| [目的] 分析 2021年河南省肿瘤登记地区女性乳腺癌发病与死亡情况及 2010—2021年发病与死亡变化趋势。[方法] 收集整理2021年河南省55个肿瘤登记处上报且质控合格的女性乳腺癌发病与死亡资料,分城乡、年龄别计算其发病/死亡粗率、中标率、世标率、构成比、0~74 岁累积率。采用Joinpoint 5.4.0 软件计算 2010—2021年河南省肿瘤登记地区女性乳腺癌中标发病率和死亡率的年度变化百分比(annual percent change, APC)和平均年度变化百分比(average annual percent change, AAPC)。[结果] 2021年河南省肿瘤登记地区女性乳腺癌的发病率为49.22/10万,中标率和世标率分别为38.52/10万和35.48/10万。乳腺癌死亡率为11.29/10万,中标率和世标率分别为7.38/10万和7.20/10万。城市地区女性乳腺癌发病率和死亡率均高于农村地区。女性乳腺癌年龄别发病率在50~54岁达到高峰,年龄别死亡率在85岁及以上组达到高峰。2010—2021年间,河南省肿瘤登记地区女性乳腺癌中标发病率呈上升趋势(AAPC=1.97%, 95%CI: 0.86%~3.21%, P<0.05),死亡率呈下降趋势(AAPC=?3.16%, 95%CI: ?5.19%~?1.11%, P<0.05)。[结论] 河南省肿瘤登记地区女性乳腺癌呈现发病率上升、死亡率下降的趋势,城乡差异仍存,提示防控重点应由单纯降低死亡转向减轻整体疾病负担,并进一步优化分层管理策略。 |
| [Purpose] To analyze the incidence and mortality of female breast cancer in cancer registry areas of Henan Province in 2021, as well as the trends in incidence and mortality from 2010 to 2021. [Methods] Data on the incidence and mortality of female breast cancer reported by 55 cancer registries in Henan Province in 2021 and meeting quality control standards were collected and collated. Crude rates, age-standardized rates by the Chinese standard population (ASRC) and the world standard population (ASRW), proportion, and cumulative rate (0–74 years) were calculated by urban/rural areas and age groups. Joinpoint 5.4.0 software was used to calculate the annual percent change (APC) and average annual percent change (AAPC) of age-standardized incidence and mortality rates of female breast cancer in cancer registry areas of Henan Province from 2010 to 2021. [Results] In 2021, the incidence of female breast cancer in cancer registry areas of Henan Province was 49.22 per 100,000, with ASRC and ASRW of 38.52 per 100,000 and 35.48 per 100,000, respectively. The mortality rate was 11.29 per 100,000, with ASRC and ASRW of 7.38 per 100,000 and 7.20 per 100,000, respectively. Both incidence and mortality rates of female breast cancer were higher in urban areas than in rural areas. The age-specific incidence rate peaked at 50–54 years, and the age-specific mortality rate peaked in women aged 85 years and above. From 2010 to 2021, the age-standardized incidence rate of female breast cancer in cancer registry areas of Henan Province showed an increasing trend (AAPC=1.97%, 95%CI: 0.86%–3.21%, P<0.05), whereas the mortality rate showed a decreasing trend (AAPC=?3.16%, 95%CI: ?5.19%–?1.11%, P<0.05). [Conclusion] Female breast cancer in Henan cancer registry areas showed an increasing incidence and a decreasing mortality, with persistent urban–rural disparities, indicating that prevention and control efforts should shift from reducing mortality alone to alleviating the overall disease burden and further optimizing stratified management strategies. |
